NACCHO is pleased to inform you that the Maricopa County Department of Public Health STD Program has been selected for a funding award as part of Identifying Promising Practices for Congenital Syphilis Prevention project. We received many strong applications, but the review committee felt your application best aligned with the goals of the project to identify local health department innovations or improvements in congenital syphilis prevention. Please note that selection is pending the following: • Agreement to NACCHO’s contract terms and conditions; • Submission of your organization’s most recent W-9 (template attached); • Completion of the Vendor Information Form (template attached); and • Completion of a call regarding review committee feedback on your application (see below). In order to indicate your acceptance of the award, please complete and email the Vendor Information Form and W-9 to rhorowitz@naccho.org by end of the day on March 20, 2020. If you are unable to accept the award, please let us know immediately. In addition, we would like to schedule a brief call with you to discuss some of the comments our review committee had about your application, as well as immediate next steps including the development of itemized budgets. Please provide your team’s availability for a 30-minute call between March 16 and April 3. Please also save the date for an all-awardees project kick-off call on March 24th at 1pm Eastern. Please be prepared to share a few sentences about your proposal to the other funded sites on that call. PLEASE NOTE: While we encourage you to share this news with others in your agency and any partners or contractors you may be engaging in this project, please do not yet publicly announce your selection. We will work with CDC and all awardees to coordinate a public announcement. We look forward to working with you on this project. If you have any questions, please do not hesitate to contact us.
