INVOCATION – INVOCACIÓN
Supervisor Sellers introduced Kevin Hartke, Mayor of Chandler and added that Mr. 
Hartke is also a Pastor at Trinity Christian Fellowship Church in Chandler.
Pastor Hartke offered the invocation.

PLEDGE OF ALLEGIANCE - JURO FIDELIDAD A LA BANDERA
Pastor Hartke led the assemblage in the Pledge of Allegiance to the flag.   
Chairman Gates thanked Pastor Hartke for the opening invocation, leading the pledge 
and for his leadership in the City of Chandler.

ROLL CALL - LISTA
The Board of Supervisors of Maricopa County, Phoenix, Arizona, convened in Formal Session at 
9:30 AM on Wednesday, October 23, 2019, in the Supervisors' Auditorium, 205 W. Jefferson, 
Phoenix, AZ 85003, with the following members present: Bill Gates, Chairman, District 3; Clint 
Hickman, Vice Chairman, District 4; Jack Sellers, District 1; Steve Chucri, District 2; Steve Gallardo, 
District 5 (entered late).  Also present: Fran McCarroll, Clerk of the Board; Kelly Gardiner, Minutes 
Coordinator; Joy Rich, County Manager; and Andrea Cummings, Legal Counsel.
Chairman Gates asked the Clerk if there were any changes or corrections on the 
agenda. The Clerk said there were two. On item #7, there was a request for a 
continuance until the November 6, 2019 meeting. This item is under Statutory Hearings 
and will require a vote for the continuance. The second item, #62, the submitting 
department asked to have the item withdrawn.

PET SHOWCASE BY MARICOPA COUNTY ANIMAL CARE AND CONTROL 
PRESENTACIÓN DE ANIMALS DOMESTICOS POR EL DEPARTAMENTO DE 
CONTROL Y CUIDADO DE ANIMALES
Ben Swan and Amanda Shane from Maricopa County Animal Care and Control 
(MCACC) were present and introduced Palmer, a senior mixed-breed dog who is 
available for adoption at the West Valley Animal Care Facility located at 2500 South 
27th Avenue Phoenix, AZ 85009. Mr. Swan said the senior dogs have been featured on 
social media and in November there will be a special event for senior dogs. Mr. Swan 
acknowledged the MCACC Field Officers who helped at the 6th Annual Fix.Adopt.Save 
Pittie-Palooza on Saturday, October 19, 2019 at the Phoenix Metro Center Mall. The 
Officers assisted with crowd control and distributed vouchers for spay and neuter 
services.
Chairman Gates thanked Mr. Swan and Ms. Shane for all they do at MCACC.

ARROYO NORTE UNITS 8, 9 & 10 SUBDIVISION ASSURANCE AGREEMENT AND 
RESOLUTION
Approve Arroyo Norte Units 8, 9 & 10 Subdivision Assurance Agreement with SLV Arroyo 
Norte, LLC and Maricopa County. This agreement serves as part of the subdivision’s 
assurance as required by Arizona Revised Statues §11-821(C) and must be approved by 
the Board of Supervisors prior to approval of the Final Plat for Arroyo Norte Units 8, 9 & 10 
(case number S2013007). This agreement to be recorded as required pursuant to A.R.S. 
§11-1101. Approve by Resolution and authorize the Chairman to execute the Agreement

Commission Recommendation: On 9/26/19, the Commission voted 7-0 to recommend 
approval of Z2018080 subject to conditions ‘a’ – ‘h’:
a. Development of the site shall be in substantial conformance with the Zoning Exhibit 
entitled “SUP for 24714 N. 11th Ave. – MC APN 210-08-012A“, consisting of 5 full-size 
sheets, dated revised July 10, 2019, and stamped received July 19, 2019, except as 
modified by the following conditions.
b. Development of the site shall be in substantial conformance with the Narrative Report

d. This Special Use Permit shall expire 10 years from the date of approval by the Board of 
Supervisors. All of the structures shall be removed within 60 days of such termination or 
expiration. If the SUP is not extended, then at the expiration of the SUP, the uses occurring 
on the property must be consistent with the underlying zoning district. 
e. The applicant/owner shall submit a written report outlining the status of the development 
at the end of 1 year from the date of approval by the Board of Supervisors. The status 
report shall be reviewed by staff to determine whether the Special Use Permit remains in 
compliance with the approved conditions. 
f. Noncompliance with any Maricopa County Regulation shall be grounds for initiating a 
revocation of this Special Use Permit as set forth in the Maricopa County Zoning 
Ordinance. 
g. The property owner/s and their successors waive claim for diminution in value if the 
County takes action to rescind approval due to noncompliance with conditions. 
h. The granting of this change in use of the property has been at the request of the 
applicant, with the consent of the landowner. The granting of this approval allows the 
property to enjoy uses in excess of those permitted by the zoning existing on the date of 
application, subject to conditions. In the event of the failure to comply with any condition, 
and at the time of expiration of the Special Use Permit, the property shall revert to the 
zoning that existed on the date of application. It is, therefore, stipulated and agreed that 
either revocation due to the failure to comply with any conditions, or the expiration of the 
Special Use Permit, does not reduce any rights that existed on the date of application to 
use, divide, sell or possess the property and that there would be no diminution in value of 
the property from the value it held on the date of application due to such revocation or 
expiration of the Special Use Permit. The Special Use Permit enhances the value of the 
property above its value as of the date the Special Use Permit is granted and reverting to

approved by the Board of Supervisors, a revised site plan shall be submitted within 30 days 
showing correct SVTs and fencing moved out of SVTs. 
b. Development of the site shall be in substantial conformance with the Narrative Report 
entitled “Desert Services International”, consisting of 11 pages, dated August 28, 2019, and 
stamped received August 28, 2019, except as modified by the following conditions.
c. The following Planning Engineering condition shall apply: 
1. MCDOT R/W permit for the driveways along W Van Buren St. will be processed through 
planning and development. The R/W permit application can be submitted with the building 
application, or provided later as part of the review process if desired. 
d. This Special Use Permit shall expire 30 years from the date of approval by the Board of 
Supervisors. All of the site improvements shall be removed within 60 days of such 
termination or expiration. 
e. Applicant shall obtain all necessary building permits pursuant to Maricopa County 
requirements. 
f. Noncompliance with any Maricopa County Regulation shall be grounds for initiating a 
revocation of this Special Use Permit as set forth in the Maricopa County Zoning 
Ordinance. 
g. The property owner/s and their successors waive claim for diminution in value if the 
County takes action to rescind approval due to noncompliance with conditions
h. Development of the site shall be in compliance with all applicable Maricopa County Air 
Quality rules and regulations.
i. Within 180 days of Board approval, the owner shall combine the parcels within the SUP 
boundary into two parcels. 
j. All outdoor lighting shall conform to the Maricopa County Zoning Ordinance.
k. Cat’s Claw (Uncaria tomentosa), Mondell Pines (Pinus eldarica), and/or Mesquite 
(Prosopis glandulosa or chilensis) shall be planted and maintained 20’ on-center along the 
Van Buren Street frontage of Lots 1 & 2, as described and depicted in the Narrative date 
stamped August 28, 2019, and along the 181st Avenue frontage. Any future outdoor 
storage within 30’ of the west perimeter property line shall be screened with Cat’s Claw 
(Uncaria tomentosa), Mondell Pines (Pinus eldarica), and/or Mesquite (Prosopis

a. Development of the site shall comply with the Site Plan entitled “Plan of Development – 
1142 N. 186TH Dr.”, consisting of 2 full-size sheets, stamped received September 16, 
2019, except as modified by the following conditions. 
b. Within 30 days of Board of Supervisors approval, a revised site plan shall be provided to 
correct the following: billboard setbacks (add rear yard and fix side yard), property setbacks 
(fix side yard), and fix lot coverage/existing buildings percentages. 
c. Development of the site shall be in general conformance with the Narrative Report 
entitled “I-10 & Perryville Rd – Application for Military Compatibility Permit”, consisting of 22

d. This Plan of Development (POD) shall only apply to the billboard land use. Any future

f. Events shall be limited from 12:00 noon to 12:00 midnight on Fridays and Saturdays and 
from 12:00 noon to 10:00 p.m. on Sundays. Installation of temporary structures for each 
event shall occur no earlier than 12:00 noon Fridays, Saturdays and Sundays only, and 
shall be fully removed by no later than 12:00 midnight. Noise generated on site shall not 
exceed 55 dB average over a 10-minute period as measured outside the boundary of the 
special use permit.
g. Outdoor parking lot lighting and wall lighting in excess of 60 watt incandescent or 
equivalent lumens shall be fully shielded and directed downward. Parking lot lighting, sign 
lighting, and wall lighting shall be turned off within 30 minutes after the event. This 
condition does not apply to outdoor light fixtures at the residential entrance to the building. 
There shall be no lighting on perimeter walls except at driveways. Light poles shall not 
exceed 15 feet.
h. Prior to the first event, the existing residence and barn shall both be properly permitted 
and modified to accommodate public assembly per 2012 International Building Code.
i. The granting of this change in use of the property has been at the request of the 
applicant, with the consent of the landowner. The granting of this approval allows the 
property to enjoy uses in excess of those permitted by the zoning existing on the date of 
application, subject to conditions. In the event of the failure to comply with any condition, 
and at the time of expiration of the Special Use Permit, the property shall revert to the

ROAD FILE DECLARATIONS - DECLARACIONES DE CARRETERA
Approve, by resolution, petitions to open and declare the following roads into the county 
highway system.  This action will serve as notice of the Board of Supervisors’ acceptance 
of all U.S. Patent easements, reservations, rights-of-way or properties along the 
alignments into the Maricopa County highway system and will also authorize the 
maintenance and acquisition of the necessary rights-of-way through donation, purchase, or 
condemnation.

In accordance with A.R.S. Titles 28- 6701, 6702 and 6703, it is recommended by the

COMMERCIAL AND INSTITUTIONAL ESTABLISHMENTS)
Pursuant to A.R.S. §49-479(b), convene the scheduled public hearing, to solicit comments 
and consider the adoption of the proposed revision of Maricopa County Air Pollution 
Control Regulations Rule 314 (Open Outdoor Fires and Indoor Fireplaces at Commercial 
and Institutional Establishments) and the associated proposed revision to the Arizona State 
Implementation Plan (SIP). Following the public hearing, the Board is requested to adopt 
the proposed amendments to the rule and to approve submission of the amended rule as a 
revision to the Arizona SIP. Upon Board approval, this item will become effective October 
23, 2019.
The Maricopa County Air Quality Department (MCAQD) is proposing to revise Rule 314 
(Open

SIP.
Rule 314 was adopted to reduce emissions of nitrogen oxides and volatile organic 
compounds (which are precursors to ozone formation), carbon monoxide, and particulate 
matter. Rule 314 identifies the types of fires that are allowed in Maricopa County, when 
each type of fire is allowed, what types of materials can and cannot be burned, and other 
requirements that are applicable to each type of fire.
The MCAQD is proposing to revise Rule 314 for the following reasons:
--To clarify the applicability and requirements of Rule 314.
--To define cooking, restrict the size of fires used for cooking, and require that fires ignited 
for cooking during a restricted burn period be extinguished once the food is suitable for 
human consumption.
--To require the use of seasoned wood in fireplaces, woodstoves, and chimineas at 
commercial and institutional establishments, and in outdoor fires ignited for the following 
purposes:
-To provide warmth for human beings;
-For recreational purposes;
-For branding of animals;
-For cooking;
-For proper disposal of the flag of the United States in accordance with 4 U.S.C. § 8.
--To incorporate existing MCAQD policies related to firefighter training during restricted 
burn periods and agricultural flame cultivation.
--To make other changes to ensure that the revised rule can be approved into the Arizona 
SIP.

AQ-2019-002-ORDINANCE P-26 (RESIDENTIAL WOODBURNING RESTRICTION)
Pursuant to A.R.S. §49-479(b), convene the scheduled public hearing, to solicit comments 
and consider the adoption of the proposed revision of Maricopa County Air Pollution 
Control Regulations Ordinance P-26 (Residential Woodburning Restriction) and the 
associated proposed revision to the Arizona State Implementation Plan (SIP). Following 
the public hearing, the Board is requested to adopt the proposed amendments to the 
ordinance and to approve submission of the amended ordinance as a revision to the 
Arizona SIP. Upon Board approval, this item will become effective October 23, 2019.
The Maricopa County Air Quality Department (MCAQD) is proposing to revise Ordinance 
P-26 (Residential Woodburning Restriction) and is proposing to submit the revised 
ordinance as a revision to the Arizona SIP.
Ordinance P-26 was adopted in response to A.R.S. § 11-871, which required Maricopa 
County to adopt and implement a residential woodburning restriction when monitoring or 
forecasting indicates that the carbon monoxide or particulate matter standards are likely to 
be exceeded.
The MCAQD is proposing to revise Ordinance P-26 for the following reasons: 
--Clarify which fires at residential locations are subject to Ordinance P–26;
--Exempt fires and appliances used for cooking from Ordinance P–26 (because A.R.S. § 
49-501(B) specifies that open outdoor fires used for cooking shall be allowed at any time);
--Clarify the ordinance by removing definitions of terms that are not used in the ordinance;
--Rephrase the ordinance to clarify the requirements that are applicable during restricted 
burn periods and the requirements that are applicable at all times;
--Reference the air quality standards in Rule 510 (so that Ordinance P–26 does not have to 
be revised each time the EPA revises a NAAQS);
--Update the methods that the MCAQD will use to notify the public when a restricted burn 
period has been declared;
--Delete the provision which stated that only violations of the ordinance which occurred 
within one year of a present offense shall be considered as prior violations (because the 
statute that specifies the consequences for violating this ordinance does not include that 
provision);
--Update the names of low income assistance programs (that are used to determine when 
low income residents may qualify for a temporary exemption) and the name of the 
implementing agency; and
--Make other changes to ensure the revised ordinance can be approved into the Arizona 
State Implementation Plan. Permit fees are not changing due to this action. (C-85-20-003-

incorporations into the Maricopa County Air Pollution Control Regulations. Upon Board 
approval, this item will become effective October 23, 2019.
The Maricopa County Air Pollution Control Regulations contain several rules, Rules 360, 
370, 371 and Appendix G, which incorporate by reference federal regulations from Title 40 
of the Code of Federal Regulations (CFR). These federal regulations include New Source 
Performance Standards (NSPS), National Emission Standards for Hazardous Air Pollutants 
(NESHAP) and Acid Rain regulations. Every July 1, the U.S. Environmental Protection 
Agency (EPA) codifies revisions made to these federal regulations during the preceding 
year. The purpose of this rulemaking is to incorporate by reference the applicable federal 
regulations as codified on July 1, 2019, and thereby update Rules 360, 370, 371, and 
Appendix G with any revisions made to the corresponding federal rules during the 
preceding year. This is necessary in order to continue to comply with the MCAQD’s 
delegated authority from EPA to implement and enforce NSPS, NESHAP, and acid rain 
programs in Maricopa County, except for those specific authorities retained by the EPA. 
After the rules are adopted by the Board of Supervisors, the MCAQD will request 
delegation authority from the EPA to implement and enforce the federal standards. These 
amendments should not have an economic impact on businesses in Maricopa County and 
should not impose additional costs on the regulated community, small businesses, political 
subdivisions, and members of the public. Permit fees are not changing due to this action.

ACCEPT FUNDING FROM US DEPARTMENT OF JUSTICE, FEDERAL BUREAU OF 
INVESTIGATION FOR THE DESERT HAWK VIOLENT CRIME TASK FORCE
Approve the Notice of Limits for FY20 State and Local Overtime Reimbursements and 
acceptance of up to $18,649 in reimbursement funding from the US Department of Justice, 
Federal Bureau of Investigation for officers assigned to FBI managed task forces. The FBI 
Desert Hawk Violent Crime Task Force agreement commenced February 1, 2018 and will 
continue for an indefinite period until terminated by one or both parties. Acceptance of this 
agreement was approved by the Board of Supervisors on February 21, 2018 under C-19-
18-046-M-00. The FY20 reimbursement funding began retroactively October 1, 2019 and 
will terminate September 30, 2020.
In accordance with paragraph 3 of the agreement, the agreement allows a 0% rate for 
indirect costs, or $0 that may be incurred by the County Attorney’s Office of Maricopa 
County for the administration of this agreement. The County Attorney’s indirect cost rate for 
FY20 has been calculated as 15.8%, or $2,946.54. The non-recoverable indirect cost of 
$2,946.54 will be covered by the department general fund. This agreement is recurring, 
non-competitive and does not require a match. The agreement does not require on-going 
cash contributions after the period end date. This funding indirectly supports the overall 
mandated function of prosecution of criminal cases.

assigned to the FBI Desert Hawk Violent Crime Task Force. The purpose of the agreement

space for the East Mesa Justice Court located at 4811 East Julep, Suite 128, Mesa, AZ.
Amendment No. 5 to Lease Agreement L-7015 extends the term of the agreement three (3) 
years. The lease term extension will begin on January 1, 2020 and expire on December 31, 
2022, unless terminated earlier as provided for in the agreement. Both parties have the

In order to obtain greater flexibility in the use of Court Commissioners, the Superior Court 
customarily has all Court Commissioners appointed as Superior Court Judges Pro

Approve the acceptance of grant funds from Arizona State Department of Homeland 
Security, (FFY19 SHSGP, 190205-01), in the not-to-exceed amount of $2,820 for the 
purpose of continuing satellite data service on the regional mobile satellite to provide 
backup network connectivity. The grant period for this grant is October 1, 2019 through 
September 30, 2020. The Maricopa County Department of Finance has calculated the 
Department of Emergency Management's composite indirect cost rate for FY20 is 10%. 
Total grant indirect costs are estimated to be $282.00 and are not recoverable. Emergency 
Management will absorb these indirect costs within their general budget.
The grant award is non-recurring, the department has to apply every year however, we 
have received the grant for the past four years. The department does not have an in-kind 
match requirement for this grant. The grant award is not a mandated function but it is the 
department responsibility to provide backup satellite communications for the county 
Emergency Operations Center and other locations within Maricopa County. The grant is 
awarded through the Arizona State Homeland Security Department- State Homeland 
Security Grant Program (SHSGP), which is a competitive grant program. Maricopa County 
jurisdictions compete for projects based off the State grant initiatives. Maricopa County has 
spearheaded the grant for the mobile satellite for the use during outages within Maricopa 
County or disaster occurring outside Maricopa County. Each year Maricopa County applies 
for recurring cost of the satellite data plan. Maricopa County does not have to fund any 
additional costs for the program.

Services Department (MCHSD). 
The purpose of this Agreement is for DES to provide $50,000 to the County for the 
provision of services related to weather related to temporary emergency shelter services to 
adults experiencing homelessness in and around the Human Services Campus (the 
Campus) located at 204 S. 12th Avenue, Phoenix, Arizona 85007. The Agreement shall 
begin on the date of the last signature and shall end on June 30, 2020.
DES has previously contracted with the County for homelessness services, however the 
contract has expired. DES awarded the Agreement to the County through a non-
competitive process.
The Human Services Department is utilizing the provisional indirect rate of 22.2% by the 
U.S. Department of Health and Human Services for FY2020 for salaries and employee 
related expenses. The total Agreement amount is $50,000 of which $0 is for salaries and 
benefits, therefore, the total estimated indirect costs are $0. 
Receipt of the funds from DES does not require in-kind or match funds and no future or 
ongoing contributions by the County at the end of the Agreement term.
The services provided under this Agreement are not a mandated function but provides a 
benefit to homeless individuals in and around the Human Services Campus.

AMENDMENT TO THE AGREEMENT WITH ARIZONA DEPARTMENT OF HOUSING 
FOR WEATHERIZATION PROGRAM SERVICES
Approve financial Amendment No. 1 to the Agreement between Arizona Department of 
Housing (ADOH) and Maricopa County administered by its Human Services Department. 
The purpose of this Amendment is for ADOH to provide an increase in funds for the 
Weatherization Assistance Program.
ADOH will provide the County with an additional $33,000 of Southwest Gas (SWG) funding 
($3,000 Admin funds, $23,096 in Program Operation funds, and $6,904 in Health and 
Safety funds). The Agreement funding ceiling shall not exceed $119,219. The funding 
period of availability is January 1, 2019 to December 31, 2019. The funds will be utilized to 
provide weatherization measures in dwellings occupied by low-income households in 
Maricopa County outside of the City of Phoenix and the City of Mesa.
ADOH contracts with the County on an annual reoccurring, non-competitive basis for 
service delivery of the Weatherization Assistance Program.

The Parties agree to enter into this Amendment No. 3 to the Agreement to amend the 
Agreement as follows:
A. Add an additional Work Statement hereinafter referred to as “2019 Work Statement” for 
the Subrecipient to construct two (2) single family homes and provide homebuyer 
assistance to low income families. All work performed or cost incurred or expended for 
2019 Work Statement shall be reimbursable through September 30, 2023.
B. The County shall provide the Subrecipient with $227,655.22 for the 2019 Work 
Statement activities. 
C. Upon approval of Amendment No. 3, the Agreement funding amount shall be increased 
to $833,320.22.
D. Section IV, Compensation, Subsection D, Reimbursement shall be amended to 
reference total funding of $833,320.22.
The County shall reimburse the Subrecipient on a Net 0 payment standard.
This Agreement is subject to A.R.S. §38-511. The foregoing paragraphs contain all the 
changes made by this Amendment. All other terms and conditions of the original 
Agreement and previous Amendments remain the same and in full force and effect as 
approved and amended.
The purpose of the Amendment is to ensure affordable housing is available in the City of 
Peoria. This Amendment is funded by a grant from the U. S. Department of Housing and 
Urban Development (HUD), (C-22-17-028-3-00). The housing activities meet all HOME 
Investment Partnerships Program requirements for the required period of affordability. 
Maricopa County is the lead agency for the Maricopa HOME Consortium.
The FY 2019-2020 Annual Action Plan contains specific projects for funding to implement 
the goals of FY2015-2020 Five-Year Consolidated Plan and serve as guides for the 
expenditure of HUD funds allocated to Maricopa County. The FY2019-2020 Annual Action 
Plan was approved by the Board of Supervisors on May 22, 2019. The proposed project for 
the City of Peoria was listed in the FY2019-2020 Annual Action Plan submitted to HUD. 
The FY2015-2020 Five Year Consolidated Plan (C-22-15-056-3-00) identifies housing and 
non-housing community development needs of low and moderate-income people in the 
Maricopa County service area (Urban County and HOME Consortium). The Five Year Plan 
establishes goals; and describes performance measures for meeting the goals. The Board 
of Supervisors approved the Five-Year Consolidated Plan on May 6, 2015.
Funding is included in the FY2020 budget and does not impact the General Fund.

Approve Amendment No. 2 to the financial Intergovernmental Agreement (IGA) between 
Arizona Department of Economic Security (DES) (DI19-002202) and Maricopa County 
administered by its Human Services Department (HSD) Workforce Development Division. 
The purpose of the Amendment is for the County to continue to administer Workforce 
Innovation and Opportunity Act Title I (WIOA) program activities in Maricopa County, 
excluding the City of Phoenix.
The Amendment addresses the following:
1. Increases Program Year 2019/Fiscal Year 2020 (PY19/FY20) funding by $29,556. 
2. The reimbursement ceiling for the Agreement shall now be increased from $30,817,466 
to $30,847,022.
DES has contracted with the County for WIOA program activities since 2014 on an annual 
reoccurring, non-competitive basis. DES has contracted with the County for more than 24 
years, which includes previous federal legislative iterations.
The period of availability for PY19/FY20 funding is April 1, 2019 through June 30, 2020.
Funding expenditure time frames vary and are dependent upon the Catalog of Federal 
Domestic Assistance (CFDA) WIOA Title I assigned to the funding. Under this Amendment 
the CFDA and related funding category shall be: 
1. Adult-CFDA 17.258 $10,853;
2. Youth-CFDA 17.259 $14,340;
3. Dislocated Worker/RR-CFDA 17.278 $4,363.
Funds under this Agreement are provided by the Workforce Innovation and Opportunity Act 
(WIOA) Title I federal program from the U.S. Department of Labor. As such, the funds are 
subject to all the provisions of Public Law 113-128 as well as any guidance issued by the 
U.S. Department of Labor and DES.
The Human Services Department utilizes the provisional indirect rate of 22.2% by the U.S. 
Department of Health and Human Services FY2020 for salaries and employee related 
expenses. The total Amendment funds are $29,556 of which $0 is for salaries and benefits. 
The total estimated indirect costs are $0.
This Agreement does not require in-kind or cash match contributions, and no future or on-
going contributions are required after the Agreement ends.
Services provided under this Agreement, are not a mandated function, but provides a 
benefit to Maricopa County residents by providing career and training opportunities, 
assistance to job seekers, youth, and to employers that need to fill vacancies. Services 
help to establish a stronger workforce in the local area. 
Maricopa County, outside the city of Phoenix, is a designated Local Workforce 
Development Area (LWDA) as designated by the Governor of Arizona. The Maricopa 
County Board of Supervisors are the designated chief elected officials for this Agreement.
Grant revenues are not local revenues for the purpose of the constitutional expenditure 
limitation, and therefore, expenditure of the revenues is not prohibited by the budget law. 
This Agreement does not alter the budget constraining expenditures of local revenues duly 
adopted by the board pursuant to A.R.S. 42 -17105. The overall grant budget will be 
adjusted as necessary to accommodate the agreement funding through a future

AMENDMENT TO THE AGREEMENT WITH WILDFIRE FOR UTILITY ASSISTANCE 
PROGRAM
Approve Amendment No. 1 to the Agreement between Wildfire formerly known as Arizona 
Community Action Association and now dba as “Wildfire” and Maricopa County, 
administered by its Human Services Department. 
The Amendment funding amount is $21,499.
The Purpose of the Amendment is to address the following changes to the Agreement: 
1. To increase the Arizona Public Service Crisis Bill Assistance funds by the amount of 
$17,509.00.
The funding increase shall now be: $52,310 for Direct Services and $10,462 for Program 
Delivery. The new allocation will be $62,772.
2. To increase the Salt River Project Bill Assistance funds by the amount of $1,540.00.
The funding increase shall now be: $113,259 for Direct Services and $11,326 for Program 
Delivery. The new allocation will be $124,585.
3. To increase the Southwest Gas Energy Share Bill Assistance funds by the amount of 
$30.00.

4. To increase the Utility Repair Replacement Deposit funds by the amount of $2,420.00.

5. To remove expenditure timeline limit of Arizona Public Service Crisis Bill Assistance 
funds. The original Agreement restricted the use of a portion of the funds for the period of 
July 1, 2019 to December 31, 2019 and the remainder to be used for the January 1, 2020 
through June 30, 2020. Through this Amendment, the full APS Crisis Bill Assistance funds 
can be expended through the Agreement term of July 1, 2019 through June 30, 2020 
without limitations.
Unless otherwise noted herein, all other provisions of the original Agreement will remain in 
place for the duration of the original Agreement.
Wildfire contracts with the County on an annual basis to administer the utility deposits and 
bill assistance for eligible low-income residents in Maricopa County outside of the City of 
Phoenix and City of Mesa.
The Human Services Department approved provisional indirect rate for FY2020 is 22.2% 
from the U.S. Department of Health and Human Services for salaries and employee related 
expenses. The total Agreement amount is $21,499 of which $0 is for salaries and ERE. 
The total recovered allowable indirect cost is $0.
Receipt of the funds from ACAA does not require in-kind or match funds and no future or 
ongoing contribution by the County at the end of the Agreement term.

(NEEF) and accept the awarded Sponsorship in the amount of $2,200.
In accordance with A.R.S. §42-17106(B), authorize the following amendments to the FY 
2020 budget: approve an appropriation adjustment to Parks and Recreation Department 
(D300) Parks and Recreation Grant Fund (230), Non-Recurring Non-Project (NRNP), 
increasing the FY2019-2020 revenue budget by $2,200 and the expenditure budget by 
$2,200. 
This is a onetime Sponsorship with no matching funds required and no further contribution 
required. The Sponsorship is not a mandated function but provides a benefit to the citizens 
through volunteer opportunities. This is a non-competitive Sponsorship. The indirect cost 
recovery rate for Parks & Recreation is 31.07%. The unrecoverable indirect cost

Maricopa County (County), a political subdivision of the State of Arizona, through its 
Department of Public Health (MCDPH) and H.S Medical Billing Services “dba” 
PractiSynergy (“Contractor”) for the purchase of medical billing outsourcing services. The 
term of the contract is for three (3) years, beginning December 8, 2019 and ending 
December 7, 2022. The cost of the services are not expected to exceed $500,000 per year 
of the three (3) year contract for a term total of $1,500,000.00. The County may, at its 
option and with the agreement of the Contractor, renew the term of this Contract up to a 
maximum of three (3) additional years, (or at the County’s sole discretion, extend the 
contract on a month-to-month basis for a maximum of six (6) months after expiration).

AMENDMENT TO LEASE AGREEMENT WITH HF1-D PHOENIX 1706, LLC
Approve and authorize the Chairman to execute Amendment No. 1 to Lease Agreement L-
7453 with HF1-D Phoenix 1706, LLC, a Delaware limited liability company (Landlord). 
Since Lease will require administrative action from time to time to carry out its intent, 
delegate authority and responsibility to either the Deputy County Manager for Maricopa 
County or the Real Estate Director for Maricopa County each individually and separately, 
for proper administration of this Lease.
Maricopa County (Tenant), administered by its Public Health Department, Office of Vital 
Registration and Immunization Divisions, utilizes 4,702 rentable square feet of office space 
located at 1850 North 95th Avenue, Suite 182, Phoenix, Arizona. Amendment No. 1 to 
Lease L-7453 extends the term of the Lease 88 months (7 years 4 months) beginning 
February 1, 2020 through May 31, 2027. The gross modified rent and other lease 
expenses will average approximately $12,000 per month over the 88 month lease term. 
Amendment No. 1 also modifies SNDA and Estoppel Certificate provisions and exhibits, 
Tenant liability coverage and early termination provisions, and Notice addresses for all

AGREEMENT AMONG THE BOARD OF SUPERVISORS, CLERK OF THE BOARD OF 
SUPERVISORS AND THE RECORDER
Approve the Elections Operations Agreement (EOA) among the Maricopa County Board of 
Supervisors, Clerk of the Board of Supervisors and the Maricopa County Recorder. This 
agreement outlines the intentions of each party regarding the management of election-
related activities in Maricopa County. The agreement is effective upon the signature of all 
parties. (C-06-20-203-3-00)
Supervisor Chucri thanked Chairman Gates, Supervisor Gallardo, Scott Jarrett and the 
Recorder’s Office for all the work that has been done to update Maricopa County 
Elections. He stated it has been a monumental task transitioning from an agreement 
developed in 1955 to an agreement appropriate for 2019.
Supervisor Gallardo concurred with Supervisor Chucri’s comments and thanked all who 
assisted in restructuring the Elections process. He stated he has been in Elections and 
understands the magnitude of the task to ensure the integrity of elections and the right 
to vote is preserved. Best in class is the goal for Maricopa County.
Chairman Gates thanked all of his colleagues especially Supervisor Gallardo for his 
bipartisan contribution. He thanked District 3 staff and the Election’s staff for their hard 
work on the item. With the 2020 elections coming, he stated that “the world will be 
watching”.

Amy Farner, former volunteer in 2016 and former employee of Maricopa County Animal 
Care and Control (MCACC). Ms. Farner stated she loved her job as a volunteer 
coordinator. Despite her compassion for the animals at the shelter, Ms. Farner left after 
seven months because of the unhealthy workplace, the lack of credibility toward the 
Director and unprofessional leadership. 
Erika Edmark former volunteer for MCACC. Ms. Edmark made referenced the 
Workplace Assessment report that was conducted in June 2019. The report used the 
word “autocratic” to describe the department leaders at MCACC. Ms. Edmark said Mary 
Martin, Director of MCACC, has a weak and ineffective team. She stated there is a 
small group of volunteers dubbed “friends of Mary” that support Ms. Martin’s leadership 
style. She presented a copy of the Assessment Report for each of the Board members.
Lorena Bader, former volunteer for MCACC. Ms. Bader stated the Mejorando Group 
Workplace Assessment was a scathing report of a dysfunctional and toxic organization. 
Ms. Bader said she was singled out for speaking up regarding personnel issues at 
MCACC. A social media post in January resulted in a private meeting between Ms. 
Bader and Ms. Martin. During the meeting, Ms. Martin made disparaging comments 
about volunteers.
 
Andrea Alden, former volunteer for MCACC. Ms. Alden stated a Workplace Assessment 
report was recently released. A two minute video summarizing the report was an 
attempt to provide damage control. Mary Martin, MCACC Director has been focused on 
public perception, numbers of live releases, and her own reputation. The low morale 
has led to staff burnout and high turnover. 
Kathleen Bau, former volunteer of MCACC. Ms. Bau was a Registered Nurse in Home

Health and Hospice Care for 20 years. During her career, she learned a code of ethics 
regarding money. Ms. Bau said during her time as a volunteer, she handled money that 
was not deposited into the proper accounts, witnessed the mishandling of money from 
donations, events and front desk fees. Ms. Bau said her experience at the shelter 
occurred prior to the current complaints being heard.
Patricia Yates, resident of Maricopa County. Ms. Yates referred to the Workforce 
Assessment Report and stated her concern for the workplace culture at MCACC. She 
said the dysfunctional management negatively impacts employee performance, group 
cohesion and organizational effectiveness. As a member of the community, a taxpayer, 
and an animal advocate, Ms. Yates said she does not understand why the Board of 
Supervisors have not taken the steps to correct the situation at MCACC.

Supervisors'/County Manager's summary of current events

Joy Rich, County Manager announced the Maricopa County Wellness Event, 5K Fun 
Run and Walk scheduled for October 24, 2019 at the Durango Complex located at 
South 27th Avenue and West Durango Street in Phoenix. Registration begins at 6:30 am 
with a start time at 7:00 am.
Supervisor Gallardo congratulated the City of Phoenix for the opening of Fry’s Food and 
Drug Store on October 23, 2019 located at 100 East Jefferson Street in downtown 
Phoenix. Supervisor Gallardo announced the Gila Bend Unified School District had the 
ground breaking ceremony on October 23, 2019 for a new school. The historic building 
they have been using is nearly 40 years old and will be replaced thanks to a school 
bond and a bond over-ride that recently passed. Supervisor Gallardo stated that 
October 24, 2019 is Wear Purple to Work Day in support of Domestic Violence 
Awareness month. He said helping those affected by domestic violence is an important 
contribution in our community.
Supervisor Sellers recently attended and reported on the following: 

-He met Tony Staffaroni, Community Relations Manager for Central Arizona 
Project. Supervisor Sellers stated it is valuable to have a ready contact with this 
key element here in our region.

-He attended a productive meeting at Senator Sean Bowie’s office with 
Councilman Sal DiCiccio’s Chief of Staff, Sam Stone and Ahwatukee residents 
in attendance. Arizona Department of Transportation representatives were 
present to discuss community concerns regarding the new south Mountain 
Freeway set to open December 2019.

-At the World Transportation Summit on October 16-18, 2019, Supervisor 
Sellers spoke with Legislators and other County Supervisors to discuss the 
importance of transportation for the future of our economy and lifestyle. Jennifer 
Toth, Maricopa County Director of Transportation spoke on technology’s place 
in transportation planning.

-East Valley Partnership Transportation Committee meeting October 22, 2019 
at Chandler City Hall. Arizona Department of Transportation (ADOT) attended 
and discussed the project updates of Interstate 17 from Anthem to Sunset Point, 
planning for the unimproved Interstate 10 between Chandler and Tucson, South 
Mountain Freeway and statewide funding issues related to maintaining our 
roadway systems. 
Chairman Gates welcomed back his Chief of Staff, Laura Etter who has been on 
maternity leave. He announced the upcoming County Coffee November 7, 2019 at Deer 
Valley Airport Restaurant 702 W. Deer Valley Drive, Phoenix. Chairman Gates guest

AGREEMENT WITH UNITED STATES GEOLOGICAL SURVEY FOR WATER 
RESOURCES INVESTIGATIONS
Approve Intergovernmental Agreement (IGA) FCD 2019A007, between the United States 
Geological Survey [Agreement #20ZFJFA0600 (USGS)] and the Flood Control District of 
Maricopa County (District), for the period of October 1, 2019 to September 30, 2020, for 
collection of stream flow data on local watersheds throughout Maricopa County. The total 
cost of the IGA is $363,620. The District’s costs under this agreement will be $214,800, 
with the USGS contributing the remaining $148,820. Payments from the District are to be 
made quarterly.
The agreement covers costs associated with the maintenance, collection, and publication 
of stream flow data and river reach characteristics at nine continuous stream flow 
monitoring stations and 22 peak flow monitoring sites throughout Maricopa County. This 
agreement further will develop and evaluate long term flood histories on watercourses in 
Maricopa County, and provide information critical to the determination of the risk of flooding 
in these watercourses. The information generated by the USGS’s ongoing work is vital to 
the safety of Maricopa County residents, and to the continuing hydrologic studies 
performed by the District. This agenda item impacts all Supervisorial Districts. (C-69-20-

AMENDMENT TO IGA WITH CITY OF PHOENIX FOR 27TH AVENUE AND OLNEY 
AVENUE DRAINAGE IMPROVEMENTS PROJECT
Approve Amendment No.1 (Intergovernmental Agreement (IGA) FCD 2018A011A) to IGA 
2018A011 (C-69-19-003-3-00) for 27th Avenue and Olney Avenue Drainage Improvements 
Project (PROJECT), between the City of Phoenix (CITY) and the Flood Control District of 
Maricopa County (DISTRICT). The estimated total cost of components associated with this 
IGA is $6,773,000 with the cost shared at 50/50%. This Amendment No.1 shall become 
effective when executed by all parties.
Due to the urgency of implementing the basin construction portion of this PROJECT prior 
to the start of the 2019 monsoon season, the original IGA will be amended so the CITY can 
undertake the construction of the three drainage basins on 23rd Avenue ahead of the 
proposed 72-inch storm drain construction. 
Currently the IGA has the DISTRICT performing the construction of the basins. This

amendment will have the DISTRICT provide the CITY the project design for the basins and 
for the CITY to lead the construction of the basins. 
In addition, the Amendment will identify that the CITY has decided not to continue to 
pursue the Federal Emergency Management Agency (FEMA) Grant and also the Durango 
Regional Conveyance Channel (DRCC) credit will be adjusted. 
The PROJECT PARTNERS agree that it would be advantageous for CITY to acquire all 
necessary rights-of-way for the PROJECT within CITY jurisdiction.

IMPROVEMENT PROJECT SEGMENT FROM 54TH AVENUE TO 25TH AVENUE
Authorize the advertisement of the Invitation for Bids for the Arizona Canal Diversion 
Channel (ACDC) Fence Improvement Project from 54th Avenue to 25th Avenue (Project 
[#118]) and award of the contract to the lowest responsible bidder, if the bid is not more 
than ten percent (10%) over the engineer’s estimate.
The District has identified deteriorating iron fencing along much of the ACDC on both the 
north and south walls for which fence post improvements are necessary to provide fall 
hazard protection and safe use of the adjacent public multi-use trail.
Proposed work for this project consists of repairing and/or replacing deteriorated and failing 
concrete pilasters and steel fence posts throughout this segment of the ACDC. This

FEDERAL EMERGENCY MANAGEMENT AGENCY GRANT FUNDING FOR VEKOL 
WASH FLOODPLAIN DELINEATION STUDY
Accept Federal Emergency Management Agency (FEMA) Cooperating Technical Partners 
Grant Funding that is anticipated to offset the full cost of the Flood Control District of 
Maricopa County’s (District’s) Vekol Wash Floodplain Delineation Study (FEMA Grant No. 
EMF-2019-CA-00017). The total amount awarded is $303,100. The term of this award is 
August 1, 2019 to July, 29, 2022. 
Flood Control’s indirect rate for Fiscal Year 2020 is 11.6%. This grant does not allow for 
indirect cost reimbursement. Unrecoverable indirect costs associated with this grant are 
estimated to be $35,160. The grant award is a one-time award. The department has no 
match requirement. The grant award supports a mandated function. The grant award is 
competitively awarded. If awarded, the department will absorb any costs beyond the grant 
award.
Authorize the Director of the Flood Control District of Maricopa County (District), or their 
designee, to process obligating and acceptance documents as needed. The grant will 
cover the cost of the study as well as travel for a required FEMA training session. 
Additionally, approve an increase to the Fiscal Year 2020 Flood Control District (D690) 
Grant Fund (989) Non-Recurring/Non-Project (NRNP) revenue and expenditure budgets in 
the amount of $103,100.00.
The Vekol Wash Floodplain Delineation Study will identify up to 30 linear miles of 
floodplains along Vekol Wash and its tributaries. Vekol Wash is located in the southwest 
portion of the county. The area is one of the last areas within Maricopa County to be 
evaluated for flood hazards and development interest in the area has begun to increase.
