AMENDMENT NO. 5 TO THE AGREEMENT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND COMMUNITY BRIDGES INC. I. Maricopa County (“County”), administered by its Human Services Department and Community Bridges Inc. (“Subrecipient”), entered into a financial Subrecipient Agreement (“Agreement”), which was fully executed on or about June 19, 2019, for the provision of Navigation services for justice-involved people experiencing homelessness in Maricopa County. The County provided the Subrecipient with $132,000 in Community Development Block Grant (CDBG) funds for the services. The Agreement contained a Start Date of July 1, 2019, and a Termination Date of June 30, 2020. The Parties fully executed Amendment No. 1 on or about February 19, 2020, to add $4,865.47 in funds for a total Agreement amount of $136,865.47. The Parties fully executed Amendment No. 2 on or about April 20, 2020 to increase the Agreement funding by $25,000 in Public Health funding to a not to exceed amount of $161,865.47, extend the Agreement term through June 30, 2021, and replace wording in the Scope of Work. The Parties fully executed Amendment No. 3 on or about July 28, 2020, to provide the Subrecipient with $132,000 in PY2020 CDBG Funds and a period of funding availability of July 1, 2020, through June 30, 2021. The Parties fully executed Amendment No. 4 on or about July 7, 2021 to extend the Agreement term through June 30, 2022 and provided an additional $132,000 in CDBG Program Year/Fiscal Year (PY/FY 2021/2022) funding and $98,6555 in Coronavirus Aid, Relief and Economic Security Act (CARES Act) (Pub. L. 116-136) (CDBG-CV3) funding, provided by the U.S. Department of Housing and Urban Development (HUD). The total available funding under Amendment 4 not to exceed amount of $230,655 for a period of July 1, 2021, through June 30, 2022. II. The Parties agree to enter into this Amendment No. 5 to amend the Agreement as follows: A. Extend the Termination Date from July 1, 2022, through December 31, 2022. B. Revise Section III (Work Statement) Paragraph A (Scope of Work), to add an additional Scope of Work hereinafter referred to as a “Outreach Scope of Work”, attached and incorporated into the Agreement. The Outreach Scope of Work activities include outreach and navigation services for individuals who are experiencing homelessness or at risk of becoming homeless in the West and East Valley of Maricopa County. Services are intended to mitigate crises, assist with diversion, and connect to long-term preventative and chronic condition care management. Add that the County shall provide the Subrecipient with an additional $424,854.24 in County General Funds Program Year/Fiscal Year (PY/FY 2021/2022) funding (Outreach Budget) for the Outreach Scope of Work activities. Funding availability period is from December 1, 2021 through December 31, 2022. III. Section II above contains all the changes made by this Amendment No. 5. All other terms and conditions of the Agreement and previously approved Amendments shall remain the same and unchanged and in full force and effect as executed by the Parties. IV. The Parties have authorized the undersigned to execute this Amendment No. 5 on their behalf, and it shall be effective upon approval and signature by both Parties. Outreach Scope of Work Community Bridges, Inc. (CBI) will work collaboratively to serve West and East Valley residents who are experiencing homelessness or at risk of becoming homeless. CBI will provide outreach and navigation services to mitigate crises, assist with diversion and connect to long-term preventative and chronic condition care management. The targeted outreach project will focus on two main service strategies: (1) CBI providing outreach to connect members to the diverse services in the community. CBI has a broad continuum of services that can be utilized to stabilize members, but we also connect members to our community partner resources. (2) CBI will work closely with community partners in the targeted areas to identify the service gaps. CBI will assist in bridging those services and to assist our partners in working with the members experiencing homelessness. West Valley Outreach: CBI will provide four (4 FTE) outreach navigators that will work in teams of two navigators. The two West Valley outreach teams will cover the West Valley communities that include Avondale, Buckeye, El Mirage, Glendale, Goodyear, Litchfield Park, Peoria, Surprise, Tolleson, and Youngtown, with an expectation that CBI will serve a minimum of 300 members annually. To provide coverage 7-days a week Outreach Team #1 is scheduled for Sunday- Wednesday from 8am – 6pm. Outreach Team #2 is scheduled Wednesday-Saturday from 8am – 6pm. The staggered schedules allow for extended coverage seven days a week. East Valley Outreach: CBI will provide two (2 FTE) outreach navigators that will work together as one team. The two teams will cover the East Valley communities Gilbert and Apache Junction. CBI will serve a minimum of 150 members annually in the East Valley. The one outreach team will provide services Monday through Friday from 8am – 4pm. The CBI outreach supervisor will be the main point of contact for the outreach teams to identify and develop relationships with community partners. Additionally, for both West and East Valley community partners, the CBI outreach supervisor will provide on-call assistance to ensure a team is dispatched upon notification by community partners, local law enforcement and emergency personnel. Calls received after hours will be addressed by the outreach teams the following day. CBI will also work with community partners on accessing after hour resources. CBI will provide outreach and engagement into Integrated Health Care services. CBI will work closely with first responders in the West and East Valley cities and Police Departments to identify high utilizers. CBI promotes community relationships within the Criminal Justice setting and assists in removing barriers for people experiencing homelessness with multiple misdemeanors that revolve around homelessness such as trespassing, shoplifting, and disorderly conduct. When outreach is involved in connecting individuals experiencing homelessness it will allow for local Police Departments and first responders to respond to other issues in the community. CBI operates several programs that provide long term support for homeless individuals, both serious mental illness (SMI) and general mental health. Through CBI's internal referral system, other CBI programs receive and refers clients to programs for long term support, based on the best fit for the client. For example, CBI operates rapid re-housing housing and permanent supportive housing programs in Phoenix and Mesa for Veterans and non-Veterans. CBI’s Outreach Team deliver a full range of services and works with clients who are frequent users of the crisis system. Through internal referrals to additional CBI programs, clients can transition to a system of care that supports them in achieving a stable life free from homelessness. The work of the outreach teams supports the work of first responders and helps to transform neighborhoods by conducting outreach in public places where homeless residents typically live. CBI's Outreach Navigators specifically address the emergent and life-threatening impact on an individual who needs treatment for their substance use addiction, other behavioral health emergency, or a combination of substance use and behavioral health (co-occurring disorders). Substance abuse and behavioral health conditions are potentially life threatening when left untreated, especially when compounded by the emotional and physical stress of homelessness. By providing early intervention, assessment and transition support, the outreach teams ensure that adults who need crisis care have the appropriate level of treatment in the most cost-effective setting. They also free up police, fire, and EMT resources to focus on public safety, and leave emergency rooms available to address true medical emergencies by decreasing the number of people waiting for behavioral health services in emergency rooms. CBI Navigation Outcomes: The Outreach Navigator outcomes include seven measures: (1) Number of adults experiencing homelessness (2) Number of adults experiencing homelessness contacted and engaged in case management process (3) Number of positive housing exits for those who are case managed (4) Number of homeless adults who achieved one or more non-housing goals (5) Number of adults referred to CBI mental health programs (6) Number of adults connected to benefits (7) Number of transports to CBI facility Contract No. C-22-19-039-3-05 Outreach Budget Page 5 of 5 Community Bridges, Inc.
