Central Ave. 3rd floor Phoenix, Arizona 85004 Re: Amendment to Grant Agreement (“Grant Agreement”) by and between The Kresge Foundation (“Kresge”) and Maricopa County Human Services Department (“Organization”) Dear Bruce: Your Organization entered into a Grant Agreement with Kresge on or about Driving Systems Change Through 2Gen & Family-Centered Approaches, with Grant number G-1901-277573. Kresge and Organization, collectively referred to as “Parties,” now wish to amend the Grant Agreement as set forth herein (the “Amendment”). This Amendment to the Grant Agreement is entered into as of the date first written above. Except as otherwise stated herein, capitalized terms shall have the meanings assigned to them in the Grant Agreement. The Grant Agreement is hereby amended to the extent necessary, to provide the following: This financial amendment to Maricopa County Human Services Department will support their ongoing efforts to advance social and economic mobility with an intentional focus on racial equity. This is one of ten requested investments to the National NextGen Initiative (NNGI) Cohort II as we celebrate the end of the learning program. The NextGen Cohort II identified shaping the future of how the human services field can lift up the respect, power, autonomy, agency, and dignity of the people they serve. Cohort members and some of their front-line staffers shared their innovations, specific practices, ideas, and inspirations. This support will enable cohort members to implement a range of identified action steps. Except as set forth herein, all other terms and conditions of the Grant Agreement shall remain in full force and effect. Original Grant Amount: $510,000 Amended Grant Amount: $520,000 Revised Grant Terms: 08/01/2019 – 12/31/2021 Next Report Requirement Due: 3/31/2022 In witness whereof, the Parties have executed this Amendment as of the day and year first written above.
