Lease No. P-50311 Maricopa County WIC Renewal C-86-18-022-M-04 Page 1 of 2 RENEWAL OF MARICOPA COUNTY (WIC) LICENSE AGREEMENT This renewal of the Maricopa County Revocable License Agreement as amended (Renewal) is made and entered into by and between the City of Scottsdale, an Arizona municipal corporation (City), and Maricopa County, a political subdivision of the State of Arizona (Licensee), collectively referenced as the Parties. A. The City and Licensee are parties to Contract No. 2017-142-COS a Revocable License Agreement (Original Agreement) for use of the Paiute Neighborhood Center to provide human services to Scottsdale residents, as amended by Amendment No. 1 on April 6, 2020 in Contract No. 2017-142-COS-A1 (First Amendment), Renewal on October 19, 2020, and Amendment No. 2 on March 2, 2021 (Second Amendment). The Original Agreement, First Amendment, Renewal, and Second Amendment are collectively referenced as the Agreement. B. The City Contract Administrator and the Licensee’s Contract Administrator (each as defined in Section 11.0 of the Agreement) have mutually determined that the Agreement’s term which would otherwise expire on October 31, 2021, should be extended an additional one-year period as provided for in the Original Agreement at Section 3.0. C. The term of this Agreement, therefore, is extended an additional one-year period to continue until October 31, 2022, unless terminated as otherwise provided in the Agreement. D. The Parties recognize that the Agreement provides for an increase in the Use Fee and the Utility Fee as set forth in Sections 6.2.1 and 6.2.2. Licensee agrees to pay such increased fees as shown in the chart below during this Renewal: Term Fixed Monthly Utility Fee Use Fee Rate Use Fee Monthly Use and Utility Fee Annual Months 49-60 $50.98 $9.00/s.f. $107.25 $1,898.76 plus rental tax E. Upon this Renewal, the terms and conditions of the Agreement shall remain in full force and effect.
