AMENDMENT TO PHPQ CONTRACT 21005 WITH SOUTHERN ARIZONA
AIDS FOUNDATION FOR RYAN WHITE PART A PROGRAM
Approve amendment 2 to PHPQ Contract 21005 with Southern Arizona AIDS Foundation for Ryan White
Part A Program Emergency Financial Assistance/Emergency Housing Payment Program.
I.
The contract is increased by an additional $1,200,000 for a cumulative not-to-exceed amount of
$2,636,825. The contract term ends June 30, 2022.
II. The Scope of Work is amended as follows:
2.1.4.2. is amended to read: If the requested amount is greater than $1,700.00, Contractor must
receive additional approval from RWPA program staff prior to making the payment.
2.1.4.3.1 is added: Motel vouchers are to be issued to those recently released from
incarceration or clients who are experiencing homelessness, with a plan to house within 60
days.
III. All other terms and conditions remain in full force and effect.
