About the Grant Program Application:

The Salt River Pima-Maricopa Indian Community 12% Gaming Grant Program (“Program”) application
will serve to provide a comprehensive review of the programming requests for Arizona cities, towns,
counties, non-municipal public service organizations, and non-profit organizations selected to apply for
grant funding. It is not the intent of the Program to award grants that would benefit individuals, be used
as sponsorships, be used for religious activities, primarily fund the indirect or administrative costs of a
program/organization, or be used for the direct benefit of the Salt River Pima Maricopa Indian
Community.

The Arizona Diamondbacks Foundation was formed in 1997. The mission of the Arizona Diamondbacks
Foundation is to support three main areas of need: homelessness, indigent healthcare and children’s
programs of all types including education, youth baseball and softball field renovations, outreach and

equipment. In addition, the Foundation has created strategic programs that address the needs of our
community including veterans, police, firefighters and teachers.

The Arizona Diamondbacks Foundation Board of Directors includes 7 appointees from the Arizona
Diamondbacks and 6 appointees from Maricopa County. The Foundation’s leadership includes Executive

Director Debbie Castaldo and Director of Operations Tara Trzinski.

Project Name: D-backs Fun at Bat

Provide a brief description of the project being submitted to the 12% SRPMIC Grant program. In the
description provide a summary of the relevant experience, administrative activity, and program
development related to the grant request.

The Arizona Diamondbacks Foundation is teaming up with Major League Baseball and USA Baseball to
promote active and healthy lifestyles to elementary school children. D-backs Fun at Bat is an entry-level
bat and ball program designed for use in elementary school physical education (P.E.) classes. The
initiative promotes a fun and active lifestyle for children while teaching them the fundamentals skills of
bat and ball sports.

Childhood obesity now affects 1 in 5 children in the United States and children in low-income areas are
more likely to be obese than their higher-income counterparts. Participation in sports has been shown to
not only improve overall health in children and adolescents, but also increase their confidence levels and
enhance their academic success.

The D-backs Fun at Bat program is designed to bring an active curriculum to at-risk children where they
are —at school! At a time when schools are facing tight budgets and less access to unique programs, the
D-backs Fun at Bat program is free for schools and school districts to adopt and includes the lesson plans
plus all the necessary equipment for grades K-5. Each participating school will receive online access to
the lessons designed in collaboration with SHAPE America, a D-backs Fun at Bat banner, a Champion
Principles Book for each child plus a Franklin equipment kit that includes bats, balls, tees and bases.

The goal in 2022 will be to have the program implemented in twenty (20) Title One schools in the
metropolitan Phoenix area. With approximately 500 children at each school, the Arizona Diamondbacks
Foundation and the Salt River Pima-Maricopa Indian Community have the potential to improve the
health and well-being of up to 10,000 children.

The D-backs Fun at Bat Program provides an opportunity to introduce thousands of students to a new
active sport while simultaneously teaching them life skills such as sportsmanship, perseverance and
confidence.

What percentage of the grant request are Direct Costs of the programming and Indirect Costs,
respectively?

All of the funds will be used directly for program expenses such as equipment, student workbooks and
curriculum distribution.
