Central Ave., 3rd Floor, Phoenix, AZ 85004 Organization History: The Maricopa County Board of Supervisors (BOS) is the governing body for the County; and appoints the County Manager who oversees County functions and is responsible for the operations of over 40 County departments, including the Maricopa County Human Services Department (MCHSD). What is now known as the MCHSD originated in 1965 and provides a wide array of services in Maricopa County for vulnerable populations and persons with barriers, including low-income families, persons seeking employment opportunities, and communities in need. Five program divisions in MCHSD are responsible for coordinating and delivering services for the most vulnerable residents of Maricopa County. Central Ave. 3rd Floor, Phoenix, AZ 85004 Phone: 602-372-1528 Fax: 602-506-2613 Email Address: Rachel.Milne@maricopa.gov Project Name: Homeowner Rehabilitation for Veterans with Disabilities Provide a brief description of the project being submitted to the 12% SRPMIC Grant program. In the description provide a summary of the relevant experience, administrative activity, and program development related to the grant request. Maricopa County is dedicated to providing housing services designed to address home livability, safety concerns, energy efficiency, and accessibility upgrades. Maricopa County Human Services Department (MCHSD) proposes a program that provides home rehabilitation that is suitably adapted to Veterans with disabilities. After home rehabilitation, Veterans will be able to age comfortably in place by living independently in a safe and accessible home. This program will be implemented using the existing Maricopa County Home Improvement Program (MCHIP) guidelines and procedures. If the Veteran is eligible for additional services, MCHSD will leverage federal and local funds to expand upon the rehabilitation. MCHSD currently has 52 families on its rehabilitation waitlist, 16 of those families are Veterans hoping to obtain assistance in making the repairs needed to maintain a healthy and safe home. MCHSD will coordinate all community referrals for this program, as well as maintain an up-to-date waitlist for assistance. MCHSD also utilizes an online inquiry system hosted on www.Maricopa.gov which allows interested Veterans to complete and submit a brief form to inquire about services available to them. To be eligible for the program, Veterans must own their home in Maricopa County; be active, honorably discharged or retired members of the armed services; have a disability; and, whenever possible, the household annual income not exceed 80% of Area Median Income. MCHSD will provide housing rehabilitation services to Veteran homeowners with physical or cognitive disabilities including, but not limited to, the following types of improvements: accessibility fixture retrofits in bathrooms and kitchens; addition of hand rails; doorway widening; reduction of trip hazards to alleviate mobility issues; as well as ramp connection and ramp modification in order to increase structure mobility, stability, and safety of homes. All MCHIP improvements are made by licensed contractors. MCHSD utilizes its community-based partnerships currently in place for efficient procurement of licensed contractors and to ensure projects are completed in a timely manner. MCHSD will further ensure the success of this program by utilizing currently employed and trained staff with client coordination and home rehabilitation experience including working with Veterans and people with disabilities. Key administrative functions will be completed by staff including: the continued review of policies and procedures, applicant intake and approval, contract management, activity tracking in online management systems, site-visits, and grant reporting. Whenever possible, MCHSD will leverage existing federal funding per home to maximize the benefits to the eligible homeowner by expanding home improvements and including additional necessary repairs, as well providing home weatherization services to increase energy efficiency. Detailed Budget: A detailed budget is attached. Amount of Total Project: $109,910 Amount of 12% Request: $75,000 (No Multi-Year Request will be accepted) If the amount of funding received is less than the amount needed for the project, do you have other funds to complete the project? (Please Explain): No. The program will not continue after December 2021. The success of this kind of program depends on continued sources of funding available each year. Maricopa County intends to continue to seek additional funding which will eliminate gaps in this service for Veterans. While federal funding is available to leverage and augment the program and maximize benefits to the homeowners, the funding is not sufficient to carry out the program. Maricopa County hopes the SRPMIC will consider supporting this program to expand and build upon this important work. Maricopa County Human Services Department received $143,000 for calendar year 2016. A total of five Veterans with disabilities were assisted with home rehabilitation. This outcome exceeded the original goal by two homes due to strategically leveraged federal grant funding. The Department received $75,000 for calendar year 2018 which provided home rehabilitation to three Veterans. The Department received $75,000 for calendar year 2019 which provided home rehabilitation to three Veterans. The Department received $75,000 for calendar year 2020 which provided home rehabilitation to three Veterans. In addition, the Department received $75,000 for calendar year 2021 which is still in process and will exceed its goal and provide home rehabilitation to four Veterans. List any other 12 % grant funds your organization may be seeking from other Tribal Governments in 2021-2022. Maricopa County Human Services Department has not requested funds from any other Tribal Governments for the Calendar Year 2022 cycle. The Housing and Community Development Division of MCHSD is the administrator for approximately $9 mil of U.S. Department of Housing and Urban Development (HUD) and $2.5 mil of U.S. Department of Energy (DOE), U.S. Department of Health and Human Services (HHS), and local funds annually. MCHSD also applied for and received approximately $1.7 mil of HUD funding to ameliorate lead-based paint hazards in pre-1978 homes that will be spent during 2020-2023. Funding for project/programs may be based on available funding.
