Contract No. C-22-21-132-X-01 Amendment No. 1 Page 1 of 2 Arizona Fire and Medical Authority AMENDMENT NO. 1 TO THE INTERGOVERNMENTAL AGREEMENT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND THE ARIZONA FIRE AND MEDICAL AUTHORITY I. Maricopa County (“County”), administered by its Human Services Department, entered into a financial Intergovernmental Agreement (“Agreement”) with the Arizona Fire and Medical Authority (“Subrecipient”) on or about June 23, 2021. The purpose of the Agreement is for the Subrecipient to install an electrical generator at the Tonopah Fire Station. The County has provided the Subrecipient with $107,374 in U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) funds. The Agreement term began on July 1, 2021 through June 30, 2023. The County and the Subrecipient collectively are referred to as the “Parties.” II. The Parties agree to amend the Agreement by this Amendment No. 1 as follows: A. Revise Section 3 (Work Statement) Paragraph 2.0 (Scope of Work) as follows: 2.1 Project Description: The Subrecipient shall purchase and install a back-up generator for the Tonopah and Whittman fire stations. 2.3 Project Beneficiaries: The project directly supports the public health and safety of 1,105 Tonopah area residents living in low-to-moderate income households. The fire station supports more than 300 square miles, including Tonopah as well as Interstate 10. The Whittman Fire Station provides public health and safety to 3,007 housing units across 35-square miles. Both fire stations have approximately 20% of the service population with disabilities. B. Increase the Agreement amount by $107,374 from $107,374 to $214,748 to purchase and install a back-up generator for the Whitman Arizona fire station, revising Section IV (Budget and Compensation) Paragraph 1.0 (Budget) as follows: Activity Budget Construction $214,748.00 Total $214,748.00 C. Funding for Amendment No. 1 is provided by U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) funds. III. The Agreement is amended to incorporate the changes contained in this Amendment No. 1. All other terms and conditions of the Agreement shall remain unchanged and in full force as approved and amended. IV. The Parties have authorized the undersigned to execute this Amendment No. 1 on their behalf, and it shall be effective upon approval and signature by both Parties.
