AMENDMENT TO CONTRACT
Between MARICOPA COUNTY
By and Through DEPARTMENT OF PUBLIC HEALTH
And MADISON ELEMENTARY SCHOOL DISTRICT
The above named contract is hereby amended as specified below:
I.
CONTRACT TERM: beginning September 1, 2021 and ending June 15, 2022.
II.
DEFINITIONS, page 11:
A. Department: refers to the Maricopa County Department of Public Health (MCDPH) and
includes the Maricopa County Office of Nutrition and Active Living
G. Stipend: refers to funds allocated to the school Wellness Coordinator and Nutrition
Manager
III.
PURPOSE page 11:
Add: Another purpose of this IGA is to provide funding to the Nutrition Manager to
implement the Smarter Lunchroom Movement throughout Madison School District, located
at 5601 N. 16th St. Phoenix, AZ 85016. The county agrees to provide funding for this
agreement from its AZ Health Zone – Maricopa County Public Health SNAP-Ed grant.
Madison School District agrees to distribute the funds to Nutrition Manager and work with
AZ Health Zone Program Coordinator on requirements.
IV.
Page 11 Add Nutrition Manager Requirements:
•
Meet with your AZ Health Zone Program Coordinator, principal, other administrators, and
project allies to discuss preliminary steps and details.
•
Plan monthly trainings with AZ Health Zone Program Coordinator to inform and make
changes in schools promoting the Smarter Lunchroom Movement (SLM) goals and objectives
and making the healthy choice the easy choice.
•
Complete the SLM scorecard and email copies to your AZ Health Zone Program
Coordinator.
•
Monthly trainings to the cafeteria managers and staff at each school within the district and
provide follow up at managers meeting
•
Assist cafeteria staff in implementing changes including putting up healthy messaging posters
and signs in the cafeteria. Make the changes to your cafeteria with the cafeteria manager,
school, and your AZ Health Zone Program Coordinator.
•
Complete another SLM scorecard.
•
Share SLM projects and highlights during local wellness team meetings.
•
Meet with your AZ Health Zone Program Coordinator to create an action plan on how to
keep the SLM going in the following school year.
•
Include the SLM implementation stories or highlights on newsletter, website, Facebook or
any form of media outlet for the school.
V.
COMPENSATION: update section A and add C and D:
A stipend in the amount of $1500 will be paid to school district for the 2021-2022 school year
and will be distributed directly to the School Wellness Coordinator.
C. Nutrition Manager Stipend:
The stipend in the amount of $1500 will be paid to school district for the 2021-2022 school
year and will be distributed directly to the Nutrition Manager.

D. Contractor agrees to act as a pay agent and pass through funds in the amount of $1500 to the
Nutrition Manager.
