I. The above-named contract is hereby amended as specified below:
A.
Contract expiration date shall be changed to April 30, 2023.
B.
Attachment A, Pricing Sheet, TOTAL AMOUNT is changed to $570,000. The
additional $350,000 is for the purpose of continuing COVID-19 related
physician outreach services.
II. All other terms and conditions of the original contract shall remain in full force and
effect.
