AMENDMENT No. 2 To MEMORANDUM OF UNDERSTANDING Between CARE1st HEALTH PLAN OF ARIZONA & MARICOPA COUNTY, ARIZONA Care1st Health Plan of Arizona Agreement for the purpose of establishing collaborative protocol executed May 8th, 2019 WHEREAS, County and Contractor have agreed to renew this Agreement. NOW, THEREFORE, the parties hereby agree to the following provision: The Agreement shall be renewed for a term from May 8th, 2021 and ending May 7th, 2022. ALL OTHER TERMS AND CONDITION REMAIN UNCHANGED. If there is any conflict between the terms of the Agreement and this Amendment, in all such events the terms of this Amendment shall control. This Amendment is subject to cancellation pursuant to A.R.S. § 38-511.
