FIRST AMENDMENT TO  
INTERGOVERNMENTAL AGREEMENT (IGA) NO. C-86-21-074-X-00 
BETWEEN MARICOPA COUNTY AND PEORIA FIRE-MEDICAL DEPARTMENT 
FOR MASS IMMUNIZATION SERVICES AND ADMINISTRATION 
 
1. The above-named Agreement is hereby amended as specified below: 
 
a. The project period is extended from July 1, 2021 through March 31, 2022. 
 
b. Additional funding in the amount of $50 ,000 for a cumulative not-to-exceed amount of 
$100,000 for the total project period of October 12, 2020 through March 31, 2022. 
 
c. In addition to Flu and other seasonal vaccines, COVID vaccinations will be provided. County 
and its contractors will abide by manufacturer’s instructions for storage, transport, handling, 
and reconstitution 
 
d. All eligible individuals, with an appointment, are to be vaccinated. Insurance status is 
irrelevant. 
 
e. Effective January 1, 2020, the COVID vaccine rate shall be $38.78 per dose, regardless of 
whether 1st or 2nd dose. 
 
f. 
Contractor: 
i. 
should pull the 6th dose from the vial if possible. Partial doses cannot be used. 
ii. must be available during scheduled vaccine delivery 
iii. must ensure only those eligible per MCDPH instruction are vaccinated. Exceptions 
include: 
a. reconstituted, end of day vaccine may be used to vaccinate staff or others not 
meeting the minimum criteria 
b. vaccine that has not been reconstituted must be returned to MCDPH per 
established procedure 
iv. must return completed patient consent forms, via email or courier, on day of 
vaccination, as daily reporting is required. 
v. must complete daily end-of-day report through the established vendor 
management map. 
 
g. MCDPH: 
i. 
will provide the needed vaccine and supplies necessary for vaccination: needles, 
syringes, alcohol pads, needle information card, vaccination cards, disposable face 
shields, surgical masks 
ii. due to availability concerns, MCDPH cannot guarantee to provide band aids, gloves, 
and other PPE. 
iii. will bill on behalf of the Contractor through the Arizona Partnership for 
Immunizations (TAPI). Contractors may choose to be responsible for their own 
billing. 
iv. will reimburse Contractor for under/uninsured individuals.

2. All other terms of the IGA remain unchanged. 
 
Should funding become unavailable, Section 4.0 of the Contract, Availability of Funds, will apply, 
allowing for a change or termination of the contract.
