FIRST AMENDMENT 
TO AGREEMENT FOR FORENSIC NURSE EXAMINER SERVICES 
BETWEEN  
 MARICOPA COUNTY, BY AND THROUGH THE MARICOPA COUNTY ATTORNEY’S 
OFFICE AND  
HONORHEALTH ON BEHALF OF ITS FORENSIC NURSE EXAMINER DEPARTMENT 
 
This first Amendment to the Agreement, Maricopa County, through the Maricopa County Attorney’s 
Office (“MCAO”) and HONORHEALTH (“Contractor”) (collectively referred to as the “Parties”), is 
effective upon approval by the Maricopa County Board of Supervisors. 
 
RECITALS 
 
WHEREAS, the Parties entered into an Agreement for professional services for medical evaluation and 
examination in cases of child and other sexual abuse, effective July 1, 2023 (“Agreement”); and 
 
WHEREAS, under Section 3.0 of the Agreement, the Parties may extend the term of the Agreement for 
an additional two (2) years; and 
 
WHEREAS, the Parties now desire to exercise that extension option; 
 
NOW, THEREFORE, in consideration of the mutual promises contained herein, the Parties agree to 
amend the Agreement as follows: 
 
1. AMENDMENT TO SECTION 3.3. – AGREEMENT TERM 
 
SECTION 3.3. - AGREEMENT TERM is hereby amended to reflect the exercise of 
the extension allowed under Section 3.4, and the Agreement term is extended for an 
additional one (1) year. 
 
The Agreement, previously set to expire on June 30, 2026, is extended through June 30, 
2027. 
 
All terms regarding services, obligations, and compensation shall remain in effect during 
this extended period unless modified by a future written amendment. 
 
SECTION 3.4. - OPTION TO EXTEND remains in effect and is referenced herein 
solely to document the Parties’ exercise of the one‑year extension option. 
 
County and FNE may extend the period of this Agreement for one (1) additional year 
beyond the original ending date of June 30, 2026. HonorHealth shall be notified by MCAO 
of its intention to extend this Agreement period at least 30 calendar days before the original 
Agreement period expires. FNE shall thereafter promptly respond if it also desires to 
extend this Agreement. 
 
  
One additional one (1) year extension is authorized by this Amendment.

Except as expressly amended by this Amendment, all other terms and conditions of the 
Original Agreement shall remain in full force and effect.
