AMENDMENT TO

MARICOPA COUNTY BY AND THROUGH ITS DEPARTMENT OF PUBLIC HEALTH
and
WESTERN REGIONAL MEDICAL CENTER, INC. D/B/A CITY OF HOPE PHOENIX 
For 
COMMUNITY HEALTH NEEDS ASSESSMENT REPORT
I.
For good and valuable consideration acknowledged by the Parties, the above-named
Contract is hereby amended to extend contract to September 30, 2026.
Section I, General Provisions, Section 16 – Right to Extend Contract, the Contract may be 
extended by mutual written and signed agreement of the Parties for additional terms up to 
three-years each.
II.
All other terms and conditions of the original contract shall remain in full force and effect.
