WHEREAS, Maricopa County (“County”) and Health Choice AZ (“Contractor”) have entered into an Agreement for the 
purpose of establishing a collaborative protocol executed on June 18th, 2019 (“Agreement”). 
 
WHEREAS, County and Contractor have agreed to renew this Agreement.  
 
NOW, THEREFORE, the parties hereby agree to the following provisions: 
 
 
1. 
The Agreement shall be renewed for a term from May 17th, 2024, and ending May 16th, 2025.  
2. 
Replace "Health Choice Arizona” with “Blue Cross Blue Shield Arizona Health Choice” to rectify the recent 
name change.   
 
ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED. If there is any conflict between the 
terms of the Agreement and this Amendment, in all such events the terms of this Amendment shall control. 
 
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511. 
 
IN WITNESS WHEREOF, this Contract Amendment is executed on the date set forth below when executed by both
