Independent Contractor Agreement
Contract No. 07012023-24
The Independent Contract Agreement dated November 3, 2023 between Arizona Community
Action Association (dba Wildfire) and Maricopa County, administered by the Human
Services Department, Housing and Community Development Division (HCD) (hereinafter
“Contractor”) to conduct application intake and eligibility determination for utility bill assistance
and deposits is hereby amended as follows:
Amendment to:
Section I. Services and Programs – 1.2 Fund Sources; Attachment 1:
1. URRD: The Direct Service amount will decrease from $213,450.00 to
$134,166.67 (portion of which will be eligible for the HVAC Waiver Pilot:
26,833.33) and the Program Delivery amount will decrease from
$42,690.00 to $26,833.33 (portion of which will be eligible for the HVAC
Waiver Pilot: $5,366.67). The total URRD allocation amount will decrease
from $256,140.00 to $161,000.00 (portion of which will be eligible for the
HVAC Waiver Pilot: $32,200.00)
Whole Agreement:
Unless otherwise noted herein, all other provisions of the original Agreement will remain in place
for the duration of the original Agreement.
