County Treasurer shall release the remaining excess proceeds in the amount of $27,604.22 to
Claimant SPS in c/o Tiffany & Bosco, P.A., upon presentation of this Order and a fully
executed U.S. Treasury Form W-9 to the Maricopa County Treasurer and a fee of
$30.00pursuant to A.R.S. §33-812(G) at the time payment is sought from the Treasurer,

No further matters remain pending and this Court expressly directs for the entry of this

judgment which is final and appealable pursuant to Arizona Rules of Civil Procedure 54(c).
