The undersigned further acknowledges that he has visited the site and has familiarized himself with
local conditions affecting the cost of the Work at the place where the Work is to be done. The
undersigned further acknowledges that if his bid is accepted he shall be bound by the terms of the
Contract Documents.

In submitting this Bid, the undersigned agrees:

1. To furnish all material, labor, tools, expendable and construction equipment, and all utility and
transportation services necessary to furnish and install, in a workmanlike manner, all Work at
the Project listed herein.

2. To hold his Bid open for not less than ninety days (90) Days after the receipt of Bids and to
accept the provisions of the instructions to Bidders regarding disposition of Bid Security.

3. To enter into and execute a Contract if awarded on the basis of this Bid within ten (10) days
after Award of the Bid, and to furnish a Performance Bond, a Labor & Material Payment Bond
and certificates of insurance in accord with the General Conditions and General Requirements
of this Contract and to deliver executed Bonds and Insurance Certificates to WICKENBURG’s
representative within ten (10) working days following the issuance of the Notice of Award.

4. To utilize the subcontractors and suppliers attached to this Bid Form unless authorized by
WICKENBURG to substitute another subcontractor or supplier.

To achieve final completion within the time period stated in the Contract Documents taken
from the date of Notice to Proceed or the date otherwise established for the commencement of
Work.

The undersigned agrees that time is of the essence and WICKENBURG will suffer financial
damages due to CONTRACTOR’s failure to complete the Work within the Contract Time.

The liquidated damages shall be as follows for each calendar day beyond the Contract Time for
which Contractors shall fail to complete the Work:

Final completion - $430.00 per calendar day

The undersigned has attached the required Bid Security and other items required in the
Instructions to Bidders. The Bid Security shall become the property of Town of Wickenburg,
Arizona, as liquidated damages for the delay and additional Work caused thereby in the event
the Contract and Bonds are not executed within the time set forth above.

WICKENBURG POOL RESURFACING - SCOPE OF WORK

The Town of Wickenburg is requesting bids for the resurfacing of its outdoor pool located at Coffinger
Park, 175 Swilling Avenue in Wickenburg Arizona. The scope of work shall include, but is not limited
to the removal and disposal of the existing pool surface, applying new tile and pebble surface on the
pool. Contractor shall comply with all OSHA requirements and safety guidelines associated with the
work within this contract. Contractor is also responsible to procure and post any required permits prior
to the commencement of work. Work is to be performed Monday — Friday during approved hours. It is
the contractor’s responsibility to create a safe working environment including sectioning off working
areas.

***Work may commence at any time after notice to proceed, so that this project is completed by
February 14, 2025 (including demo, prep, installation, curing, and water treatment)

The following are the minimum requirements for this project. All bids will be for a fully delivered
resurfacing meeting the following minimum specifications and requirements. Please enter an X to
indicate understanding of the requirement and bid compliance. Any deviation or exception must be
clearly noted and explained and included with your submitted bid.

PREPARATION OF POOL SURFACE

(& The contractor will be responsible for draining the pool.

{8 Remove all existing pool surface including, pebble, waterline tile, and haul away.
(& Cut and chip out around all fittings, returns, and drains.

Inspect pool surfaces to identify conditions that might interfere with proper bonding of coating and
repair as needed.

INSTALLATION
(3 Supply and install new waterline 6x6 tile. Include skimmers, step trim, depth markers and silicone

caulk top joint.
( Supply and install new 6” x 6” (8) lane lines and targets.

(8 Supply and install new 1” x 1” non-slip tile at zero entry area.

Repair grabrail anchors. Supply and install new step handrails (2) and ladder grabrails (5).
Repair pool slide as needed.
Replace (13) existing pool lights with white LED.

Remove and replace all suction frame and grates and install new VGB ANSI/APSP/ICC-16 2017
compliant frame and grates.

All pool finish work must strictly follow manufacturer installation guidelines, references and
recommendations.

Inspection will be completed by the Town of Wickenburg to determine that work has been
performed according to specification, and workmanship, and to the satisfaction of the Town of
Wickenburg Parks and Facilities staff.
