FSL Funding Agreements (2014-2024)

SIXTH AMENDMENT TO FUNDING AGREEMENT FSL PROGRAMS, INC. 
TOWN OF WICKENBURG, ARIZONA 
This Sixth Amendment to the Funding Agreement for FSL Programs, Inc. (this 
"Amendment”) is entered into by and between the TOWN OF WICKENBURG, an 
Arizona municipal corporation ("Town"), and FSL Programs, Inc. ("Contractor"). Town 
and Contractor are collectively referred to in this Agreement as the "Parties," and 
each is sometimes referred to as a “Party.” 
RECITALS 
A. Contractor and Town are parties to that certain Funding Agreement, approved 
by the Wickenburg Town Council on June 2, 2014, through Resolution No. 
1750 as amended by the First, Second, Third, Fourth, and Fifth 
Amendments to Funding Agreement (the “Agreement”. 
B. The Parties now desire to amend the Agreement in accordance with the terms 
and conditions set forth below. 
AGREEMENT 
NOW, THEREFORE, in consideration of the mutual promises made in the 
Agreement and this Amendment, the Parties agree as follows. 
1. 
Incorporation of the Recitals. The foregoing recitals are incorporated herein by 
this reference. 
2. Term. Agreement, as amended, shall commence upon execution of both parties 
and terminate on May 17, 2024. Future Funding Agreements may be approved 
by Council on an annual basis as part of the budget process. Section 2.1 of the 
Agreement is amended to read as follows: 
Social Services and Senior Citizen Programs. Contractor shall offer social 
services and community enrichment programs to the citizens of Wickenburg 
through the operation of a Social Services Navigation office and the FSL Senior 
Center. 
3. Annual Funding. Section 3 of the Agreement is amended to read as follows: 
Responsibilities of Town. Town hereby commits up to $60,000.00 of annual 
funding for Fiscal Year 2023-24 to Contractor to meet the deliverables described 
in Section 1. Funding shall be appropriated by Council from the Town's general 
fund and delivered in a single installment to Contractor's address at 466 West 
Wickenburg Way, Wickenburg, Arizona, 85390 on or before August 15, 2023. 
The funding under this Agreement is separate and distinct from any funding that

may be available through other grants or subrecipient agreements, including the 
Community Action Agency: Subaward Agreement Between the Town of 
Wickenburg and The Foundation for Senior Living. 
4. 
No Other Revisions. All other provisions of the Agreement remain unchanged. 
IN WITNESS WHEREOF, the Parties have executed this Amendment as of the last 
date set forth below their respective signatures.
