AMENDMENT TO
INTERGOVERNMENTAL AGREEMENT
Between
MARICOPA COUNTY, Ryan White Part A Program
and
Maricopa County Special Health Care District dba Valleywise Health

I The above-named contract is hereby amended as specified below:

A. Number 3. Agreement Amount $2,000,000 annually will be deleted and replaced by:
Number 3. Agreement Amount $3,000,000 annually.
B. Section Ill Scope of Work shall be updated to include 2 new sections:

Section 1.18 Referral for Health Care and Support

DEFINITION: Referral for Health Care and Support Services directs a client to needed core
medical or support services in person or through telephone, written, or other type of
communication. Activities provided under this service category may include referrals to assist
HRSA RWHAP-eligible clients to obtain access to other public and private programs for
which they may be eligible (e.g., Medicaid, Medicare Part D, State Pharmacy Assistance
Programs, Pharmaceutical Manufacturer's Patient Assistance Programs, and other state or
local health care and supportive services, or health insurance Marketplace plans).

Section 1.19 Ending the HIV Epidemic (EHE) Infrastructure
DEFINITION: Specific to Ending the HIV Epidemic funding, this service category is reserved

for activities and costs associated with the development and expansion of data systems. It
may include technical assistance on the type, design, and building of new data systems,
bridging existing systems to achieve data integration, improving data entry to decrease
burden and increase accuracy, training of staff and providers on collecting and using data,
and employing experts to provide accurate and in-depth data analysis.

Ul. All other terms and conditions of the original contract shall remain in full force and effect.
