ACTION:  Approve a one-year contract renewal with Avesis Third Party Administrator, Inc. to provide 
a vision services program for employees and their dependents.
FISCAL IMPACT:  The total cost of this contract shall not exceed $305,000. Sufficient funds have 
been appropriated in the Tempe Health Fund, Cost Center 4167 (employees), for anticipated 
expenditures during Fiscal Year 2026/2027, contingent upon City Council approval of the City’s annual 
budget.
RECOMMENDATION:  Approve the contract renewal.
 
BACKGROUND INFORMATION:  (T22-065-01) On March 3, 2022, City Council awarded a two-year 
contract with three one-year renewal options to Avesis Third Party Administrator, Inc. for a vision service 
provider for employees and their qualified dependents. This renewal request is the fourth and final 
renewal option.
