COMMUNICATION FACILITIES IN THE RIGHT-OF-WAY STANDARD TERMS &
CONDITIONS

To Whom It May Concern:

This letter is provided on behalf of Crown Castle Fiber LLC in connection with its request to
satisfy the insurance requirements set forth in the Notice of Wireless Communication
Facilities in the Right-of-Way Standard Terms & Conditions dated 06/08/2026 the
Agreement through an established self-insurance program.

The undersigned hereby attests that Crown Castle Fiber LLC:

maintains substantial financial resources and liquidity, with financial resources
sufficient to cover claims up to $60,000 per occurrence and $60,000 in the
aggregate;

possesses sufficient net worth and operating capacity to satisfy reasonably
anticipated liabilities and obligations arising under the Agreement;

maintains an established self-insurance and/or risk management program
consistent with industry practice, covering commercial general liability, workers’
compensation, automobile liability, and employers’ liability;

remains in good financial standing; and

maintains this self-insurance program on a continuous basis, currently in effect as
of the date of this letter.

In support of this request, Crown Castle Fiber LLC has provided its most recent audited
Form 10-K filed with the U.S. Securities and Exchange Commission, which reflects the
company's financial condition, assets, revenues, and operating history.

Crown Castle Fiber LLC agrees to promptly notify the City of EL Mirage in writing should
there be any material change in its financial condition or if its self-insurance program is
discontinued or materially modified.
