El Mirage Community Partnership Grant Program
Application
Program Overview 
The City of El Mirage Community Partnership Grant supports nonprofit organizations that provide essential
health and human services directly to El Mirage residents, including low-income families, seniors, youth,
and other individuals who may benefit from additional support.  
Health and human services refer to programs, projects, or activities that improve the general health,
including physical, mental, emotional, or social well-being, of individuals and families. These services
address basic needs, promote safety and stability, and enhance residents' overall quality of life.  
The program offers one-time funding of up to $10,000 per application annually from July 1 to June 30.
Eligible nonprofits and other qualified charitable organizations with 501(c)(3) status or IRS letter of
determination recognizing nonprofit status under A.R.S. §42-11154 may submit multiple applications for
funding, provided each application represents a distinct program or project.  
Applicants are limited to one application per program or project. Each application must specify the
minimum funding required. Applications are evaluated based on an evaluation scoring rubric approved by
the City Council. The ultimate funding decisions are made by the El Mirage City Council. 
Applicant Eligibility Criteria   

The applicant must be a 501(c)(3) nonprofit corporation or other qualified charitable or nonprofit
organization as provided for in A.R.S. §42-11154. 

The applicant must provide direct health and human services to El Mirage residents and
demonstrate how the program or project improves resident well-being. 

The applicant must demonstrate the ability to generate revenue from sources other than the City of
El Mirage. 

The applicant must submit the five following required documents:
    1) Proof of 501(c)(3) status or IRS determination letter under A.R.S. §42-11154. 
    2) Organizational mission statement. 
    3) Board of directors list, including roles and affiliations. 
    4) Last two years of IRS Form 990 filings (or explanation if not available or required to file). 
    5) Completed application signed by an authorized representative. 

If awarded, the applicant must register with the City of El Mirage as a vendor and provide an IRS

Form W-9 to receive funds. 

If awarded, the applicant must demonstrate satisfactory progress toward stated goals and
objectives and provide timely and accurate reports as required by the city.

Adelante Healthcare is a community-based, nonprofit federally qualified health center (FQHC) dedicated to
improving the health and well-being of the communities it serves by providing high-quality, affordable, and
accessible healthcare services. Adelante’s mission is to improve the health of communities by delivering
innovative, patient-centered care that addresses the whole person and supports long-term wellness.
Adelante Healthcare provides comprehensive integrated services, including primary medical care,
pediatrics, women’s health, behavioral health, dental care, nutrition services, pharmacy support,
community resource navigation, and enrollment assistance for public benefits and health coverage
programs. Through its Mobile Medical Unit (MMU), Adelante extends these services directly into the
community to reach individuals facing barriers to care such as transportation limitations, mobility
challenges, financial hardship, and lack of access to providers.
Serving patients across the West Valley, including El Mirage, Adelante Healthcare focuses on underserved
and vulnerable populations, including seniors, low-income families, uninsured individuals, children, and
individuals living with chronic health conditions. Adelante’s goal is to reduce healthcare disparities, improve
access to preventive and primary care, support chronic disease management, and strengthen overall
community health through accessible, integrated, and equitable healthcare services.

Adelante Healthcare is requesting funding to support the “Adelante Mobile Medical Unit Care for Seniors”
program, which will expand access to healthcare services for seniors residing in El Mirage who face
transportation, mobility, and financial barriers to care. Through Adelante’s Mobile Medical Unit (MMU),
seniors will receive preventive health screenings, chronic disease management support, vaccinations,
telehealth access, and care coordination services directly within their communities, including senior
housing communities, assisted living facilities, and other accessible community locations.
The program is designed to improve access to primary and preventive healthcare services for older adults
with chronic health conditions such as hypertension, diabetes, heart disease, chronic kidney disease, and
chronic obstructive pulmonary disease. In addition to in-person services provided through the MMU,
seniors will have access to telehealth visits with licensed medical providers. When medically necessary,
the program will also provide prescription assistance and medication delivery support for seniors who
experience transportation challenges or financial hardship.
The MMU is staffed by Adelante Healthcare’s licensed clinical team and supported by existing telehealth
technology, care coordination services, and community partnerships. Services will focus on improving
continuity of care, increasing follow-up appointment participation, reducing avoidable emergency
department visits, and supporting seniors in managing chronic conditions more effectively.
Funding from the City of El Mirage will support direct program costs associated with delivering mobile
healthcare services to seniors, including outreach activities, telehealth connectivity, medical supplies,
patient education materials, and operational support for community-based service delivery. The program
will help ensure that seniors in El Mirage can access healthcare services in a timely, convenient, and
trusted setting while remaining connected to ongoing primary care services through Adelante Healthcare.
Describe how this program or project will benefit El Mirage residents directly and the need or gap
in service that it meets. Please include data to support your response.
Older adults often experience significant barriers to accessing healthcare services, including limited
transportation, mobility challenges, chronic health conditions, social isolation, and financial hardship.
These barriers can lead to delayed care, unmanaged chronic diseases, increased emergency department
utilization, and avoidable hospitalizations. Seniors living in underserved communities may face additional
challenges accessing timely primary and preventive care services close to home. Analysis of senior
patients at Adelante’s Surprise location, limited to patients 60 years of age and older with an encounter
within the past three years, identified 3,232 seniors who reported lack of transportation as a barrier to care.
Additionally, 5,100 senior patients have never had a telehealth visit, highlighting significant gaps in
healthcare access and digital connectivity among older adults.
The Adelante Mobile Medical Unit Care for Seniors program directly addresses these barriers by bringing
healthcare services into the community and providing care in locations that are accessible and familiar to
seniors. Through the Mobile Medical Unit, El Mirage seniors will receive preventive screenings, chronic
disease management support, vaccinations, telehealth access, prescription assistance, and referrals to

ongoing primary care services. By reducing transportation and access barriers, the program will improve
continuity of care and help seniors better manage chronic health conditions.
Evidence from Arizona demonstrates that mobile medical units are an effective strategy for addressing
healthcare access barriers, particularly for populations facing transportation and mobility challenges.
Across the state, mobile units are used to deliver essential services directly to underserved communities,
helping to overcome geographic, financial, and workforce related barriers to care. National research further
indicates that FQHC-led mobile health unit models improve primary care engagement by reducing
common barriers such as transportation, cost, and limited access to providers, all of which
disproportionately impact seniors.
The program will directly benefit El Mirage residents by increasing access to healthcare services,
improving health outcomes, supporting healthy aging, and helping seniors remain safely connected to
ongoing care within their community.

Medical Unit and clinical infrastructure are already operational, allowing services to begin quickly following

The Adelante Mobile Medical Unit Care for Seniors program will operate throughout the City of El Mirage at
accessible community-based locations, including senior housing communities, assisted living facilities,
community centers, and other sites where seniors regularly gather. Adelante Healthcare’s Mobile Medical
Unit (MMU) is based within the West Valley service area and is specifically designed to bring healthcare
services directly into underserved communities.
Providing services within El Mirage allows Adelante to reduce common barriers to care, including

transportation limitations, mobility challenges, and difficulty accessing healthcare providers. By delivering
services directly where seniors live and socialize, the program improves convenience, continuity of care,
and participation in preventive and chronic disease management services.
The mobile model also allows Adelante Healthcare to coordinate with local community partners and
respond flexibly to areas of greatest need within El Mirage. Seniors will have access to preventive
screenings, vaccinations, telehealth visits, prescription assistance, and referrals to ongoing primary care
services in trusted and familiar settings close to home.
Describe how your program or project will be open to the public and ensure accessibility for
persons with disabilities, including specific accommodations.
The Adelante Mobile Medical Unit Care for Seniors program will be open to eligible seniors residing in the
City of El Mirage, regardless of income, insurance status, or mobility limitations. Outreach and scheduling
support will be coordinated through community partnerships, senior housing communities, and local
organizations to ensure broad awareness and access to services.
Adelante Healthcare is committed to providing accessible and inclusive services for individuals with
disabilities. The Mobile Medical Unit is ADA accessible and designed to accommodate seniors with
mobility challenges. Staff will provide assistance with boarding and exiting the MMU when needed, and
services will be delivered in locations that are physically accessible to participants.
Additional accommodations include language interpretation services, patient education materials in
multiple languages, telehealth support for seniors unfamiliar with technology, and care coordination
assistance to help patients navigate follow-up care and prescription access. Adelante Healthcare also
maintains compliance with all applicable federal and state accessibility and nondiscrimination requirements
to ensure equitable access to services for all participants.

Section D: Collaboration and Outreach

Describe how you will collaborate with the City of El Mirage to coordinate outreach promoting the
program or project to the residents of El Mirage.
Adelante Healthcare currently partners with the City of El Mirage to bring convenient, no-cost health
services directly to residents at the El Mirage Senior Center. Through this ongoing collaboration, trained
Adelante staff are on-site twice each month to assist residents with healthcare enrollment, eligibility
determination, and access to community resources. Services are available on the 1st and 3rd Tuesday of
each month from 8:00 a.m. to 12:00 p.m., with walk-ins welcome and no appointment required.
As part of this partnership, Adelante Healthcare provides free health screenings and wellness education on
the first Tuesday of each month. Additional on-site services include AHCCCS and KidsCare eligibility and
enrollment assistance, SNAP application support, TANF cash assistance support, Health Insurance
Marketplace application assistance, and access to Adelante Healthcare’s Sliding Fee Discount Program,
which helps make healthcare services affordable based on income.

Building on this established partnership, Adelante Healthcare will continue collaborating with the City of El
Mirage to support outreach, community awareness, and resident engagement for the Adelante Mobile
Medical Unit Care for Seniors program. Adelante will coordinate with city departments, community
resource staff, and senior-focused programs to share information about available services, service
schedules, and eligibility requirements with residents.
Outreach efforts will include distributing bilingual flyers and educational materials, sharing program
information through city communication channels and community events, and coordinating visits at
community-based locations that are accessible to seniors. Adelante Healthcare will also work with city staff
to identify areas of greatest need and support outreach to vulnerable seniors who may face transportation,
mobility, or financial barriers to care.
This ongoing collaboration strengthens community trust, improves awareness of available healthcare
services, and helps ensure that El Mirage seniors can access timely preventive and primary care services
within their community.
How will you work with other local partners to maximize impact and ensure residents are aware of
and engaged with your services?
Adelante Healthcare will collaborate with a network of local partners throughout El Mirage to maximize the
impact of the Adelante Mobile Medical Unit Care for Seniors program and ensure seniors are aware of and
connected to available healthcare services. Key partners may include senior housing communities,
assisted living facilities, community centers, faith-based organizations, food assistance programs, social
service agencies, and other community-based organizations that regularly serve older adults.
These partnerships will support coordinated outreach efforts by helping identify seniors who face barriers
to healthcare access, including transportation limitations, mobility challenges, chronic health conditions,
financial hardship, or social isolation. Community partners will assist with distributing bilingual outreach
materials, sharing service schedules, hosting mobile unit visits, and encouraging participation among
residents who may otherwise delay or avoid care.
Adelante Healthcare will work closely with partners to provide health education, appointment scheduling
assistance, telehealth support, and referrals to ongoing primary care and supportive services. The Mobile
Medical Unit’s flexible, community-based model allows services to be delivered directly in trusted and
familiar locations where seniors already gather, helping reduce barriers and increase participation in
preventive care and chronic disease management services.
In addition to direct outreach, Adelante Healthcare will maintain ongoing communication with community
partners to evaluate community needs, identify service gaps, and coordinate resources that improve
continuity of care for El Mirage seniors. Through these collaborative efforts, the program will strengthen
community engagement, improve awareness of available healthcare services, and support healthier aging
outcomes for seniors throughout El Mirage.
Part 3: Program or Project Goals and Outcomes
Please provide up to three goals of the program or project and describe how progress will be
measured to ensure success in providing health and human services to the residents of El
Mirage. It is important to ensure that your goals correspond with the proposed number of residents to
be served within the program year. Explain how you will collect and report data and measure the
impact for each stated goal. 
Example Goal: Of the 80 persons served by the project, 40% (32 persons) will find employment within
three months of entering the program. This will be measured by follow-up contact with each program

participant. 
Measurable Goal No. 1
Provide mobile healthcare services to at least 300 seniors during the program year, including
approximately 250 El Mirage residents. Services will include preventive screenings, chronic disease
management support, vaccinations, telehealth access, and care coordination. Success will be measured
through patient encounter data, service utilization reports, and demographic tracking collected through
Adelante Healthcare’s electronic health record system and program outreach logs
Measurable Goal No. 2
Increase continuity of care and follow-up participation among seniors receiving services through the Mobile
Medical Unit. Adelante Healthcare aims for at least 75% of participating seniors to complete one or more
follow-up appointments, including telehealth or primary care visits. Progress will be measured through
appointment attendance records, referral tracking, and care coordination documentation maintained within
Adelante Healthcare’s electronic health record system.
Measurable Goal No. 3
Improve chronic disease management outcomes for participating seniors by increasing access to
preventive care and health monitoring services. Adelante Healthcare aims for at least 60% of seniors with
chronic conditions to demonstrate improved or stabilized health indicators, such as blood pressure or blood
glucose levels, during the program year. Outcomes will be measured through clinical assessments, follow-
up visits, and electronic health record data collected by Adelante Healthcare providers.

Is program or project implementation contingent upon receiving the full award?
No. Adelante Healthcare is committed to implementing the Adelante Mobile Medical Unit Care for Seniors
program regardless of the final award amount. Existing organizational infrastructure, staffing, and
community partnerships will support continued program operations. However, full funding would allow
Adelante Healthcare to expand outreach, increase the number of seniors served, enhance telehealth
access, and provide additional prescription assistance and community-based service delivery activities.
If approved for an amount less than 100% of your request, how will you address the shortfall?  
If awarded less than the requested amount, Adelante Healthcare will leverage existing organizational
resources, operational support, and community partnerships to continue program implementation.
Adelante may reduce the frequency of outreach activities, limit the number of community-based service
events, or phase in certain supportive services while prioritizing direct healthcare services for seniors.
Additional grant opportunities and in-kind community support will also be explored to help offset funding
gaps.

Adelante Healthcare will sustain the Adelante Mobile Medical Unit Care for Seniors program through a
combination of organizational operational support, community partnerships, healthcare reimbursement
opportunities, philanthropic funding, and future grant opportunities. Adelante Healthcare has an
established infrastructure that supports community-based healthcare delivery, including licensed providers,
telehealth capabilities, care coordination staff, and operational support for the Mobile Medical Unit.
The organization actively pursues local, state, federal, and private foundation funding opportunities that
support healthcare access, chronic disease management, senior wellness, and underserved populations.
Adelante Healthcare also leverages in-kind support from community partners, including outreach
assistance, event space, and collaborative service delivery opportunities. Existing healthcare
reimbursement mechanisms for eligible services will further support program sustainability and continuity
of care for seniors in El Mirage.
If the program receives less than the requested amount of funding, what adjustments, if any, would
be made to the program and services provided to El Mirage residents?
If awarded less than the requested amount, Adelante Healthcare will continue implementing the Adelante
Mobile Medical Unit Care for Seniors program by leveraging existing organizational resources and
operational infrastructure. Direct healthcare services for seniors will remain the highest priority. However,
Adelante may reduce the frequency of outreach activities, limit the number of community-based service
events, scale back marketing and educational materials, or phase in supportive services such as
incentives and expanded telehealth support. Adelante Healthcare will also seek additional funding
opportunities and community partnerships to help offset funding gaps while maintaining essential
healthcare access for El Mirage seniors.
