El Mirage Community Partnership Grant Program
Application
Program Overview 
The City of El Mirage Community Partnership Grant supports nonprofit organizations that provide essential
health and human services directly to El Mirage residents, including low-income families, seniors, youth,
and other individuals who may benefit from additional support.  
Health and human services refer to programs, projects, or activities that improve the general health,
including physical, mental, emotional, or social well-being, of individuals and families. These services
address basic needs, promote safety and stability, and enhance residents' overall quality of life.  
The program offers one-time funding of up to $10,000 per application annually from July 1 to June 30.
Eligible nonprofits and other qualified charitable organizations with 501(c)(3) status or IRS letter of
determination recognizing nonprofit status under A.R.S. §42-11154 may submit multiple applications for
funding, provided each application represents a distinct program or project.  
Applicants are limited to one application per program or project. Each application must specify the
minimum funding required. Applications are evaluated based on an evaluation scoring rubric approved by
the City Council. The ultimate funding decisions are made by the El Mirage City Council. 
Applicant Eligibility Criteria   

The applicant must be a 501(c)(3) nonprofit corporation or other qualified charitable or nonprofit
organization as provided for in A.R.S. §42-11154. 

The applicant must provide direct health and human services to El Mirage residents and
demonstrate how the program or project improves resident well-being. 

The applicant must demonstrate the ability to generate revenue from sources other than the City of
El Mirage. 

The applicant must submit the five following required documents:
    1) Proof of 501(c)(3) status or IRS determination letter under A.R.S. §42-11154. 
    2) Organizational mission statement. 
    3) Board of directors list, including roles and affiliations. 
    4) Last two years of IRS Form 990 filings (or explanation if not available or required to file). 
    5) Completed application signed by an authorized representative. 

If awarded, the applicant must register with the City of El Mirage as a vendor and provide an IRS

Form W-9 to receive funds. 

If awarded, the applicant must demonstrate satisfactory progress toward stated goals and
objectives and provide timely and accurate reports as required by the city.

At AllThrive 365, our mission is to prepare and embrace individuals to age independently, safely, and with
peace of mind. Founded in 1974 as Foundation for Senior Living we changed our name to AllThrive 365 in
early 2025. Today, we are the fifth largest healthcare nonprofit in Arizona, building a community where we
can all age with confidence. We proudly serve over 32,000 individuals annually through our work, which is
grounded in three core pillars: Health, Housing, and Connection.
Our Health pillar addresses the full spectrum of well-being, from physical and mental health to food access
and chronic disease management. This pillar ensures individuals have the tools and support needed to
thrive throughout every stage of aging.
Our Housing pillar promotes stability and dignity by ensuring people can access and remain in safe,
affordable homes. We address both immediate housing needs and long-term solutions to prevent
displacement.
Our Connection pillar strengthens community bonds and reduces isolation by fostering meaningful
relationships and providing opportunities for engagement. We prioritize social wellness as a vital part of
aging well.

AllThrive 365 is respectfully requesting City of El Mirage Community Partnership grant funds for our
successful Northwest Valley Connect Transportation Program, which is in direct alignment with the city's
mission to enhance the quality of life of El Mirage residents. By partnering with AllThrive 365, El Mirage
residents will have further access to our transportation program which will improve the general health,
including physical, mental, emotional, and social well-being of individuals in El Mirage. Our Northwest
Valley Connect Transportation Program improves the health and welfare of low-income seniors, adults with
disabilities, and veterans living in and around El Mirage. Based in Surprise and serving northwest valley
communities, including El Mirage, the program's paid staff and volunteers use AllThrive 365's fleet of six
wheelchair accessible buses and vans, as well as volunteers' vehicles, to meet a community need for safe,
affordable, door-to-door transportation for vulnerable populations. This helps ensure El Mirage residents
have consistent access to medical appointments, meals, and social gatherings at senior centers, grocery
stores, pharmacies, and more. By providing affordable or no-cost rides, the Northwest Valley Connect
Transportation Program promotes optimal health outcomes, enables participants to meet their basic needs,
reduces isolation, and helps residents feel more connected to their community by providing opportunities to
socialize with others.
A $10,000 Community Partnership Grant from the City of El Mirage will directly support driver capacity,
essential vehicle repair, and preventive maintenance costs. These funds will help ensure fleet reliability,
reduce service interruptions, and make certain of safe, on-time rides for El Mirage residents. While
AllThrive 365 currently partners with the City of El Mirage through a separate agreement supporting
specific transportation services, this Community Partnership Grant request is distinct in scope. The funds
requested through this application would specifically support vehicle repair, preventive maintenance, and
driver capacity. This investment will strengthen the overall reliability and availability of the Northwest Valley
Connect Transportation Program for El Mirage residents, ensuring continuity of service and reducing ride
denials due to mechanical or capacity limitations.
Describe how this program or project will benefit El Mirage residents directly and the need or gap
in service that it meets. Please include data to support your response.
In the City of El Mirage, the consequences of transportation gaps are immediate and compounding,
creating significant barriers to health, independence, and quality of life. Vulnerable residents' lack of
accessible, affordable transportation options which result in missed medical appointments, unmanaged
chronic conditions, and social isolation. The city lists quality of life as being of top priority for its citizens.
Quality of life and access to essential services are interconnected. Without access to essential services,
individuals - particularly seniors and individuals with disabilities - are likely to experience a poor quality of
life. Without transportation, essential care services become inaccessible. Approximately 1 in 5 adults
without transportation forgo needed medical care each year (Robert Wood Johnson Foundation, U.S.
Census Bureau). Lack of transportation also fuels social isolation. In 2023, 1/3 of older adults reported
feeling isolated or lacking companionship (University of Michigan National Poll on Healthy Aging), and

these are not just quality-of-life concerns. They are serious health risks. As former U.S. Surgeon General
Vivek H. Murthy warned, loneliness is a critical public health issue, linked to increased risk of heart
disease, dementia, stroke, depression, anxiety, and premature death. Its mortality impact is comparable to
smoking up to 15 cigarettes a day, surpassing obesity and physical inactivity. The Northwest Valley
Connect Transportation Program aims to address these needs and fill the gap in available and accessible
services for vulnerable El Mirage residents.

Where will your project or program be located, and how does this location support your ability to
serve El Mirage residents effectively?
Though headquartered in neighboring Surprise, the Northwest Valley Connect Transportation Program
regularly serves El Mirage residents, picking them up at their homes and transporting them to various
locations in and around El Mirage then returning them safely back to their homes.
Describe how your program or project will be open to the public and ensure accessibility for
persons with disabilities, including specific accommodations.
AllThrive 365 has over two decades of experience serving El Mirage residents in various capacities, and
this Northwest Valley Connect Transportation Program has operated successfully since 2014, filling a
critical community gap in services. It is the only long-standing, large-scale nonprofit transportation program
in the northwest valley providing affordable, door-to-door services with a fully wheelchair-accessible fleet
service. While a few local municipalities have started small transportation contracts, they have limitations,
primarily that services are not door-to-door and travel limited distances. Unlike those transportation
contracts, we do provide door-to-door services, reaching any vulnerable individual in need of
transportation. Furthermore, our fleet of six vehicles are equipped with state-of-the-art wheelchair access,
allowing for specific disability accommodations.

Are you currently partnered with, or have you ever partnered with, the City of El Mirage?

Describe how you will collaborate with the City of El Mirage to coordinate outreach promoting the
program or project to the residents of El Mirage.
AllThrive 365 will leverage the City's shared resources and use the City's channels to amplify outreach
messages, allowing us to reach more El Mirage residents than ever before. In partnership together, we can
ensure that the residents of El Mirage that might be impacted by limited transportation access never slip
through the cracks and receive the services they want and need to have a good quality of life.
How will you work with other local partners to maximize impact and ensure residents are aware of
and engaged with your services?
AllThrive 365 works closely with a range of local community partners including healthcare providers, social
service agencies, grocery stores, and pharmacies to help residents access transportation for essential
appointments and daily needs. While many of these partnerships are informal, they are built on
long-standing relationships through which partners are aware of our services and regularly help connect
residents to the rides we provide. This collaborative, referral-based approach allows us to reach residents
where they already receive care and services.
In El Mirage, AllThrive 365 has an established presence through frequent service to the El Mirage Senior
Center, where our transportation services are well known among participants and staff. Center staff ensure
awareness and utilization by sharing information directly with residents who may benefit from
transportation assistance. In coordination with the City, AllThrive 365 also leverages municipal
communication channels and community touchpoints to ensure residents remain informed and engaged
with available services.
Part 3: Program or Project Goals and Outcomes
Please provide up to three goals of the program or project and describe how progress will be
measured to ensure success in providing health and human services to the residents of El
Mirage. It is important to ensure that your goals correspond with the proposed number of residents to
be served within the program year. Explain how you will collect and report data and measure the
impact for each stated goal. 
Example Goal: Of the 80 persons served by the project, 40% (32 persons) will find employment within
three months of entering the program. This will be measured by follow-up contact with each program
participant.

In 2026-2027, the Northwest Valley Connect Transportation Program expects to serve 250 unduplicated,
low-income seniors, adults with disabilities, and veterans who lack transportation.

Measurable Goal No. 2
In 2026-2027, the Northwest Valley Connect Transportation Program expects to provide over 20,000 rides
to medical appointments, social gatherings and meal programs at senior centers, grocery stores,
pharmacies, and other critical community locations.

Increase program revenue sufficiently to reduce the number of requested rides we must decline. (Last
year, we had to turn down 329 requested rides due to limited funds).

Is program or project implementation contingent upon receiving the full award?
No, the program will continue with a partial award but may have to defer routine vehicle maintenance,
which can lead to vehicle downtime and reduced capacity to serve El Mirage residents. The program is
already unable to accommodate all transportation requests due to limited resources. When we receive
multiple ride requests for the same time slot, we prioritize medical appointments, which necessitates
denying an average of 27 rides a month for other purposes (grocery shopping, social activities, other non-
urgent trips). Full grant award will enable us to better meet the needs of El Mirage residents.
If approved for an amount less than 100% of your request, how will you address the shortfall?  
For over 50 years, diverse support - both monetary and in-kind support - has ensured the continuous
success of AllThrive 365's various programs. The Northwest Valley Connect Transportation Program is no
exception. We will address any shortfall how we always have, through individual donations, other
foundation grants, partner income, minimal transit fees for low-cost rides, corporate giving, and other local
municipalities.

AllThrive 365's Northwest Valley Connect Program will be sustained as it has been for years- through
individual donations, foundation grants, partner income, minimal transit fees for low-cost rides, corporate
giving, and local municipalities. We have deep connections with community partners and long-term funders
which help ensure the sustainability of our services for many more years to come.
If the program receives less than the requested amount of funding, what adjustments, if any, would
be made to the program and services provided to El Mirage residents?
If the program receives less than the requested amount of funding, AllThrive 365 will be less able to
provide rides to the amount of people we expect to. The program will still run, but the vulnerable citizens of
El Mirage will receive less transportation assistance than anticipated.
