El Mirage Community Partnership Grant Program
Application
Program Overview 
The City of El Mirage Community Partnership Grant supports nonprofit organizations that provide essential
health and human services directly to El Mirage residents, including low-income families, seniors, youth,
and other individuals who may benefit from additional support.  
Health and human services refer to programs, projects, or activities that improve the general health,
including physical, mental, emotional, or social well-being, of individuals and families. These services
address basic needs, promote safety and stability, and enhance residents' overall quality of life.  
The program offers one-time funding of up to $10,000 per application annually from July 1 to June 30.
Eligible nonprofits and other qualified charitable organizations with 501(c)(3) status or IRS letter of
determination recognizing nonprofit status under A.R.S. §42-11154 may submit multiple applications for
funding, provided each application represents a distinct program or project.  
Applicants are limited to one application per program or project. Each application must specify the
minimum funding required. Applications are evaluated based on an evaluation scoring rubric approved by
the City Council. The ultimate funding decisions are made by the El Mirage City Council. 
Applicant Eligibility Criteria   

The applicant must be a 501(c)(3) nonprofit corporation or other qualified charitable or nonprofit
organization as provided for in A.R.S. §42-11154. 

The applicant must provide direct health and human services to El Mirage residents and
demonstrate how the program or project improves resident well-being. 

The applicant must demonstrate the ability to generate revenue from sources other than the City of
El Mirage. 

The applicant must submit the five following required documents:
    1) Proof of 501(c)(3) status or IRS determination letter under A.R.S. §42-11154. 
    2) Organizational mission statement. 
    3) Board of directors list, including roles and affiliations. 
    4) Last two years of IRS Form 990 filings (or explanation if not available or required to file). 
    5) Completed application signed by an authorized representative. 

If awarded, the applicant must register with the City of El Mirage as a vendor and provide an IRS

Form W-9 to receive funds. 

If awarded, the applicant must demonstrate satisfactory progress toward stated goals and
objectives and provide timely and accurate reports as required by the city.

Briefly describe your organization, the services it provides, mission, goals, and population served. 
A safe home should never become a source of fear, yet for many low-income families, seniors, and
veterans in El Mirage, aging homes and unaffordable repairs threaten their health, stability, and ability to
remain in the community they call home.
Habitat for Humanity Central Arizona is a nonprofit organization dedicated to bringing people together to
build homes, communities, and hope. Since 1985, Habitat has partnered with families across Maricopa
County to create safe, stable, and affordable housing opportunities that strengthen neighborhoods and
improve quality of life. Habitat’s mission is rooted in the belief that everyone deserves a decent place to
live and that stable housing is foundational to health, financial security, educational success, and long-term
community wellbeing.
Habitat provides a broad continuum of housing services designed to meet families where they are and help
them remain safely housed. Programs include affordable homeownership opportunities, neighborhood
revitalization services, home preservation and repair programs, aging-in-place modifications for seniors,
critical repairs for veterans and low-income homeowners, financial education, and community engagement
initiatives.
Through the Neighborhood Revitalization (NR) program, Habitat helps existing homeowners remain safely
and stably housed by addressing critical health and safety repairs that many low-income families cannot
afford independently. Repairs often include HVAC replacement, plumbing, electrical work, roofing, flooring,
accessibility modifications, weatherization, and other urgent repairs that directly impact safety and livability.
These services are particularly important for seniors, veterans, individuals with disabilities, and families
living on fixed or limited incomes who may otherwise face displacement or deteriorating living conditions.
What sets Habitat apart is its holistic, equity-based model and its leadership along the housing continuum
in advancing affordable homeownership and long-term housing stability. Habitat does not simply repair
homes or build houses; it creates pathways for families to achieve stability, preserve generational wealth,
and remain connected to the communities they love. Families contribute sweat equity, participate in
education programs, and become active stakeholders in strengthening their neighborhoods. Habitat also
leverages volunteers, partnerships, donated materials, and community collaboration to maximize impact
and stretch every funding dollar further.
Habitat serves low-to-moderate income households throughout Maricopa County, including many
communities experiencing rising housing costs and aging housing stock. In El Mirage specifically, Habitat’s
Neighborhood Revitalization program has already served five local families through critical repairs
including plumbing, HVAC, electrical, and flooring improvements, investing approximately $30,000 directly
into preserving safe housing for vulnerable residents. Current identified repair needs for El Mirage
homeowners exceed $45,000–$50,000, underscoring the ongoing demand and the importance of
continued investment in housing stability for local residents.

Habitat for Humanity Central Arizona is requesting support from the City of El Mirage to expand critical
home repair and neighborhood revitalization services for low-income homeowners living within the city.
Funding will support Habitat’s Neighborhood Revitalization program, which helps vulnerable residents
remain safely housed by addressing urgent home repair needs that directly impact health, safety,
accessibility, and long-term housing stability.
Across El Mirage, many longtime homeowners are living in aging homes requiring significant repairs that
have become financially out of reach due to rising costs, inflation, fixed incomes, and increasing economic
pressures. Seniors, veterans, individuals with disabilities, and low-income families are often forced to live
with failing HVAC systems, unsafe electrical systems, plumbing leaks, deteriorating flooring, or other
hazardous conditions because they simply cannot afford repairs independently. Left unaddressed, these
issues can lead to worsening health outcomes, increased utility costs, unsafe living environments, and
even displacement.
Habitat’s Neighborhood Revitalization program directly addresses these challenges by providing critical
repair services that allow residents to safely remain in their homes and maintain housing stability. Repairs
may include HVAC replacement, plumbing repairs, electrical upgrades, flooring replacement, accessibility
modifications, weatherization improvements, and other critical health and safety repairs identified during

home assessments.
Habitat has already demonstrated success in El Mirage. To date in 2026, Habitat has served five El Mirage
families through the NR program, completing repairs including plumbing, HVAC, electrical, and flooring
improvements with approximately $30,000 invested directly into local homes. Current waitlisted repair
projects are estimated between $45,000 and $50,000, reflecting a substantial unmet need among El
Mirage residents.
This program is designed to create immediate and lasting impact. Critical repairs improve physical safety,
preserve affordable housing stock, reduce the financial burden on families, and help prevent displacement.
For seniors and individuals with disabilities, repairs can also improve accessibility and allow residents to
age safely in place. Beyond the individual household, revitalized homes contribute to stronger
neighborhoods, improved community pride, and greater neighborhood stability overall.
Habitat will leverage skilled staff, contractors, volunteers, donated materials when available, and
community partnerships to maximize the impact of grant funding. The program is implementation-ready
and repairs can begin quickly upon funding availability.

El Mirage residents are increasingly facing housing instability caused not only by rising housing costs, but
also by the growing inability to maintain and repair aging homes. Many low-income homeowners,
particularly seniors, veterans, and individuals living on fixed incomes, are unable to afford critical repairs
necessary to keep their homes safe and livable. While affordable rental housing receives significant
attention, there are far fewer resources available to help existing homeowners preserve the homes they
already own.
Habitat for Humanity Central Arizona’s Neighborhood Revitalization program fills this critical gap by helping
vulnerable homeowners remain safely housed through essential health and safety repairs. Without
intervention, many residents are forced to live with unsafe conditions such as failing air conditioning
systems during Arizona’s extreme heat, hazardous electrical systems, plumbing failures, deteriorating
flooring, or accessibility barriers that place residents at risk of injury or displacement.
El Mirage faces growing economic pressures that heighten the need for housing preservation services.
According to U.S. Census data, median household income in El Mirage remains below many surrounding
West Valley communities, while housing and repair costs continue to rise significantly. Additionally, many
homes in the community are aging and require ongoing maintenance that low-income households often
cannot absorb financially.
Habitat’s program directly benefits El Mirage residents by:
Preserving safe and affordable homeownership
Preventing displacement and homelessness
Supporting seniors aging safely in place
Improving health and safety conditions
Reducing financial strain on low-income households

Habitat has already seen meaningful impact locally. Through prior NR efforts, Habitat served five El Mirage
families and invested approximately $30,000 into critical repairs including HVAC, plumbing, electrical, and
flooring improvements. However, identified repair needs currently exceed an estimated $45,000–$50,000,
demonstrating that the need far surpasses available resources.
This program creates long-term community impact because preserving existing affordable housing is often
far less costly and more effective than replacing lost housing stock after families are displaced. By
investing in existing homeowners now, the City of El Mirage can help stabilize neighborhoods, protect
vulnerable residents, and preserve affordable housing opportunities within the community.

The proposed program will serve residents living throughout the City of El Mirage. Habitat for Humanity
Central Arizona’s Neighborhood Revitalization program operates directly within the homes and
neighborhoods of eligible residents, allowing services to reach homeowners where they already live and
removing transportation or accessibility barriers that often prevent vulnerable residents from receiving
assistance.
Because the work occurs directly at each homeowner’s residence, Habitat is able to efficiently address
neighborhood-level housing challenges while building trust and relationships within the community. This
approach is especially important for seniors, veterans, and residents with disabilities who may have limited
mobility or transportation access.
Habitat’s central operations within Maricopa County position the organization to effectively serve El Mirage
residents alongside neighboring West Valley communities. Habitat already has experience working within
El Mirage through previously completed repair projects and has developed familiarity with the housing

conditions and repair needs commonly impacting local residents.
The location-based nature of the program also allows Habitat to respond strategically to concentrated
areas of need and maximize neighborhood-level impact. By preserving owner-occupied homes throughout
El Mirage, Habitat helps maintain neighborhood stability, reduce visible housing deterioration, and
strengthen overall community wellbeing.
In addition, Habitat collaborates closely with municipal partners, local referral agencies, and community
organizations to identify eligible homeowners and ensure residents are connected to available services.
This coordinated approach improves outreach efficiency and helps ensure resources are directed toward
households with the greatest need.

Habitat for Humanity Central Arizona is committed to ensuring that all programs and services are

income level. The Neighborhood Revitalization program is open to qualifying El Mirage homeowners who
meet income eligibility requirements and demonstrate critical repair needs impacting health, safety, or
housing stability.
Habitat works to remove barriers that may prevent residents from accessing services. Outreach materials
and application assistance are available in multiple languages when needed, including Spanish, to better
serve the diverse population of El Mirage. Staff provide one-on-one support throughout the intake and
application process to help residents understand eligibility requirements and navigate documentation
needs.
The program is especially designed to support seniors, veterans, and individuals with disabilities through

Habitat for Humanity Central Arizona values strong partnerships with local municipalities and looks forward
to collaborating closely with the City of El Mirage to ensure residents are aware of available Neighborhood
Revitalization services.
Habitat will coordinate with City staff to develop outreach strategies that effectively reach eligible
homeowners, particularly seniors, veterans, and low-income households most likely to benefit from critical
repair services. Outreach efforts may include:
Sharing program information through City communication channels
Participation in community events and neighborhood meetings
Distribution of flyers and informational materials
Referrals from City departments and staff
Social media collaboration and digital outreach

Our Neighborhood Revitalization team engages with community groups and neighborhood meetings in the
areas they serve. So, with funding, that would be something they engage with.

measured to ensure success in providing health and human services to the residents of El
Mirage. It is important to ensure that your goals correspond with the proposed number of residents to
be served within the program year. Explain how you will collect and report data and measure the
impact for each stated goal. 
Example Goal: Of the 80 persons served by the project, 40% (32 persons) will find employment within
three months of entering the program. This will be measured by follow-up contact with each program
participant.

No. Habitat for Humanity Central Arizona will move forward with the Neighborhood Revitalization program
regardless of the final award amount. However, the level of funding received will directly impact the
number of El Mirage households that can be served and the scope of repairs completed.
Full funding would allow Habitat to address a greater portion of the currently identified repair needs within
El Mirage, where waitlisted repairs are estimated between $45,000 and $50,000. Reduced funding may
require Habitat to prioritize only the most urgent health and safety repairs or serve fewer households
during the program period.

If approved for an amount less than 100% of your request, how will you address the shortfall?  
If we receives less than the full requested amount, the organization will work to address the shortfall
through a combination of additional fundraising efforts, in-kind donations, volunteer labor, corporate
partnerships, and other grant opportunities.
Habitat regularly leverages donated materials, skilled
volunteers, and community partnerships to maximize available funding and reduce project costs wherever
possible. In some cases, repairs may also be phased or prioritized based on urgency to ensure the most

plumbing failures, accessibility modifications, and unsafe flooring conditions would remain the highest

In some cases, Habitat may phase repairs over time or reduce the number of completed projects in order
to stay within available funding levels. The organization would also continue seeking supplemental support
through additional grants, donations, volunteer labor, in-kind materials, and community partnerships to help
offset funding gaps and maximize available resources.
Habitat’s collaborative and cost-effective service model helps stretch limited funding as far as possible. By

leveraging volunteers, donated materials, and community partnerships, Habitat can often reduce project
costs while still delivering meaningful impact for residents.
Even at a reduced funding level, the program would continue providing critical assistance that helps El
Mirage residents remain safely housed, preserve affordable homeownership, and improve overall housing
