El Mirage Community Partnership Grant Program
Application
Program Overview 
The City of El Mirage Community Partnership Grant supports nonprofit organizations that provide essential
health and human services directly to El Mirage residents, including low-income families, seniors, youth,
and other individuals who may benefit from additional support.  
Health and human services refer to programs, projects, or activities that improve the general health,
including physical, mental, emotional, or social well-being, of individuals and families. These services
address basic needs, promote safety and stability, and enhance residents' overall quality of life.  
The program offers one-time funding of up to $10,000 per application annually from July 1 to June 30.
Eligible nonprofits and other qualified charitable organizations with 501(c)(3) status or IRS letter of
determination recognizing nonprofit status under A.R.S. §42-11154 may submit multiple applications for
funding, provided each application represents a distinct program or project.  
Applicants are limited to one application per program or project. Each application must specify the
minimum funding required. Applications are evaluated based on an evaluation scoring rubric approved by
the City Council. The ultimate funding decisions are made by the El Mirage City Council. 
Applicant Eligibility Criteria   

The applicant must be a 501(c)(3) nonprofit corporation or other qualified charitable or nonprofit
organization as provided for in A.R.S. §42-11154. 

The applicant must provide direct health and human services to El Mirage residents and
demonstrate how the program or project improves resident well-being. 

The applicant must demonstrate the ability to generate revenue from sources other than the City of
El Mirage. 

The applicant must submit the five following required documents:
    1) Proof of 501(c)(3) status or IRS determination letter under A.R.S. §42-11154. 
    2) Organizational mission statement. 
    3) Board of directors list, including roles and affiliations. 
    4) Last two years of IRS Form 990 filings (or explanation if not available or required to file). 
    5) Completed application signed by an authorized representative. 

If awarded, the applicant must register with the City of El Mirage as a vendor and provide an IRS

Form W-9 to receive funds. 

If awarded, the applicant must demonstrate satisfactory progress toward stated goals and
objectives and provide timely and accurate reports as required by the city.

Briefly describe your organization, the services it provides, mission, goals, and population served. 
The Valley of the Sun YMCA (VOS YMCA) is a nonprofit, community benefit organization established in
1892. Today, VOS YMCA consists of twelve branches serving metropolitan Phoenix and Flagstaff, as well
as a residential camp in Prescott. As one of the oldest and largest providers of human services in Arizona,
VOS YMCA serves more than 75,000 individuals annually through programs focused on youth
development, healthy living, and social responsibility.
The YMCA’s mission is to put Christian principles into practice through programs that build a healthy spirit,
mind, and body for all. While mission driven, VOS YMCA is a non sectarian, non religious organization that
is inclusive of all individuals, regardless of background, beliefs, or ability to pay.
Guided by its mission and community focus, the YMCA does not turn anyone away due to inability to pay.
Through generous support from individual donors, corporations, foundations, and public partners, VOS
YMCA provides financial assistance to ensure broad access to programs and services. This approach
allows the YMCA to support individuals and families facing financial hardship, social disengagement, and
limited access to high quality community resources.
VOS YMCA delivers a wide range of programs designed to address identified community needs, including
meal programs, preschool and early learning, afterschool and day camps, childcare for working families,
youth workforce development, Safety Around Water education and swim lessons, diabetes prevention,
STEM education, Camp Sky Y residential camp, wellness and outreach programs for seniors and
veterans, exercise programs for cancer survivors, and housing support services for individuals
experiencing homelessness.
Through these programs, the Valley of the Sun YMCA serves people of all ages across diverse
communities, with a focus on promoting stability, well being, and opportunity for individuals and families
throughout central and northern Arizona.
Opened in 2014 through a partnership with the City of El Mirage, the Northwest Valley Family YMCA (NWV
YMCA) serves the communities of El Mirage, Surprise, Peoria, and Glendale. The branch operates as a
community based facility providing childcare, youth programs, family services, and health and wellness
opportunities designed to meet local needs.
The Northwest Valley service area experiences lower median household income and educational
attainment compared to Maricopa County and statewide averages, with more than 15% of residents living
at or below 100% of the Federal Poverty Level. Poverty rates are higher among Hispanic or Latino, African
American, and multiracial households. Many families rely on hourly or entry level employment and face
ongoing barriers to stable childcare, education, and supportive services.
Through its programs and partnerships, the NWV YMCA works to ensure residents of El Mirage and
surrounding communities have access to reliable, inclusive, and community based services that support
family stability, workforce participation, and individual well being.

The Northwest Valley Family YMCA (NWV YMCA) provides high-quality early learning and school-age
childcare programs for ages 1-12 that ensure children are safe, supported, and thriving while their parents
work or attend school. Our programs focus on nurturing the whole child through evidence-based practices
that promote social-emotional development, academic readiness, healthy living, and character building,
and our early learning center participates in Quality First with a 3-star rating. To make these opportunities
accessible, we are requesting funding to provide scholarships for low- and moderate-income families who
otherwise could not afford the consistent, quality care their children need. Strong community partnerships
extend our reach and impact: local schools promote our programs to families, the YMCA provides
transportation to and from El Mirage, Riverview, Luke Elementary and Dysart Middle Schools for families to
participate in our program, Scottsdale Unified School District provides nutritious meals and snacks daily,
Subaru Superstore provides backpacks loaded with school supplies, and the local Sam’s Club provides
supplies for program activities. Together, these collaborations ensure that YMCA children and families not
only have access to safe, affordable care, but also to the nutrition, healthcare, and community resources
that strengthen families, promote equity, and create lasting impact in our community.

The program responds to documented and ongoing barriers faced by low- and moderate-income families
in El Mirage. Rising childcare costs, limited availability of state subsidy assistance, and increased demand
have created gaps in access to reliable care. Many families rely on hourly or entry-level employment and
cannot maintain work schedules without dependable childcare. The YMCA’s tuition assistance program
provides critical family support by reducing financial strain and supporting continued workforce
participation. In 2025, the YMCA provided more than $43,000 in childcare financial assistance to El Mirage
residents through similar funding sources and internal scholarships.
Grant funds will be used exclusively to provide direct tuition assistance for low- and moderate-income El
Mirage families. Parents or guardians complete a financial assistance application and provide verification
of household income. Eligibility is determined using an income scale aligned with the Arizona Department
of Economic Security, ensuring assistance is equitably distributed to families with the greatest financial
need. Priority is given to families experiencing financial hardship, including those who are working but
income-limited, those on waiting lists for state childcare subsidies, or those facing temporary economic
disruption. Awards are structured as partial tuition subsidies, allowing limited grant funds to support as
many families as possible while maintaining program sustainability.
Upon receiving award, funds will be integrated into the YMCA’s existing scholarship allocation system.
Families will be enrolled on a rolling basis to respond to immediate childcare needs throughout the year.
Tuition credits will be applied directly to participant accounts, and funds will be tracked by grant source to
ensure compliance and accountability.
Program participation, attendance, and financial assistance awards are tracked through the YMCA’s
secure data management system. This system allows staff to monitor enrollment, subsidy amounts, and
household eligibility while maintaining confidentiality. Aggregate, de identified data will be used to report
outcomes to the City of El Mirage, including the number of children served, total assistance distributed, and
service duration.
In addition to childcare services, families enrolled in the program benefit from the YMCA’s interconnected
network of supports, including referrals to community resources, access to family programming, and
connection to additional YMCA services when appropriate. These coordinated supports enhance family

The YMCA’s Tuition Assistance Program directly benefits City of El Mirage residents by increasing access
to licensed, reliable childcare for low-income families who would otherwise be unable to afford care.
Affordable childcare is a critical support for parents who are working, seeking employment, or pursuing
education, and for children who benefit from safe, structured early learning environments. By reducing
financial barriers, this program supports both family stability and economic participation within the El
Mirage community.
El Mirage families face significant economic challenges that directly impact childcare access. The median
household income in the Northwest Valley service area is approximately $51,416, which is lower than both
Maricopa County and Arizona averages. More than 15% of residents live at or below 100% of the Federal
Poverty Level, with higher poverty rates among Hispanic or Latino, African American, and multiracial
households. Many families rely on hourly or entry-level employment and have limited flexibility when

childcare becomes unaffordable or unavailable.
The need for tuition assistance has increased due to rising childcare costs and limitations in public subsidy
programs. The Arizona Department of Economic Security has implemented waiting lists for childcare
assistance, leaving many eligible families without support. As a result, families who are working but
income-limited often fall into a service gap: they earn too much to qualify immediately for assistance, yet

By focusing tuition assistance on El Mirage residents, the program ensures that public funds directly
support local families experiencing financial hardship. The YMCA’s existing systems ensure assistance is
allocated equitably and monitored responsibly. Data is tracked on enrollment, attendance, and financial

The program will be located at the Northwest Valley Family YMCA, 12450 W. Cinnabar Avenue, El Mirage,
AZ 85335. The facility is located within the City of El Mirage and was developed in partnership with the City
to ensure YMCA services are accessible to residents across the community.
Because the site is centrally located within El Mirage, families are able to access childcare services close
to home, work, and schools, reducing transportation barriers for low income households. Locating the
program within city limits ensures that grant funds directly benefit El Mirage residents.
The Northwest Valley Family YMCA is a licensed facility designed to provide early learning and childcare
services in a safe, stable setting. Operating the program at an established YMCA location allows for
efficient enrollment, oversight, and data tracking, while offering families consistent and reliable care in a
familiar community facility.
This location allows the YMCA to effectively serve El Mirage families by combining accessibility, regulatory

The YMCA’s Tuition Assistance Program is open to the public and available to all eligible families residing

The NWV YMCA collaborates closely with the City of El Mirage to coordinate outreach and promotion of
the Tuition Assistance Program to El Mirage residents. This collaboration builds on the YMCA’s existing
agreement with the city, through which the YMCA operates programs that support city recreation and
community services, including providing subsidized access for El Mirage residents.
Through this established partnership, the YMCA and City will coordinate efforts to ensure families are
aware of available childcare and early learning support. Outreach will focus on residents most likely to
benefit from tuition assistance, including low and moderate-income working families and households
seeking affordable childcare options.
Planned outreach strategies include coordinated promotion through City of El Mirage communication
channels, such as the City’s newsletter, website, and social media platforms. These channels provide
direct access to El Mirage residents and are an effective way to share information about eligibility,
enrollment periods, and how to apply for childcare tuition assistance at the YMCA.
The YMCA will also participate in city sponsored events and community activities when appropriate to
promote early learning and childcare services. These settings allow for direct engagement with families,
opportunities to answer questions, and distribution of program information in a familiar and trusted
environment.
In alignment with the City’s goals, the YMCA will highlight the availability of financial aid and scholarships
for El Mirage residents and will continue to look for additional grant opportunities to help expand access to

childcare and early learning for families with financial need. Program staff will coordinate messaging with
the City to ensure information shared with residents is accurate, timely, and consistent.
In addition to City led outreach, the YMCA will promote the program onsite at the NWV YMCA and through
referrals from community partners serving El Mirage, including schools and local organizations. These
combined efforts ensure that outreach is broad, targeted, and responsive to community needs without
duplicating City or YMCA messaging.
Through coordinated marketing, shared communication platforms, and ongoing partnership, the YMCA and
the City of El Mirage will work together to ensure residents are informed about and able to access early
learning and childcare tuition assistance services. This collaborative approach maximizes visibility,
supports equitable access, and ensures grant funded services directly reach El Mirage families most in
need.

El Mirage residents are aware of and engaged with early learning and childcare tuition assistance services.
These partnerships expand outreach beyond YMCA facilities and allow information to reach families
through trusted, community based channels.
The YMCA continues to build and strengthen relationships with local schools, businesses, and faith based
organizations serving El Mirage residents. Schools and early learning partners are key referral sources for
families with young children and provide an effective way to share information about childcare options and
financial assistance with parents and caregivers. Local businesses and employers will be engaged to help
promote the program to working families who may be seeking affordable childcare to maintain
employment.
Faith based organizations and churches serve as additional outreach partners by sharing program
information with congregants and community members. These partners are often trusted messengers
within the community and provide opportunities to reach families who may not otherwise be connected to
formal childcare systems.
The YMCA also promotes the program directly to current YMCA members and program participants who
reside in El Mirage. Many families already engaged with the YMCA may be unaware of available tuition
assistance or changes in eligibility and will be informed through onsite communication, staff referrals, and
YMCA newsletters.
Marketing and outreach efforts includes a combination of social media, email newsletters, printed
materials, and word of mouth promotion through partners. When appropriate, the YMCA will participate in
community events, tabling opportunities, and local gatherings, including city sponsored or partner hosted
events, to provide face to face outreach, answer questions, and assist families with understanding
available services.
These efforts are coordinated to ensure consistent messaging across partners and to avoid duplication.
Program staff work with partners to share clear information about eligibility, enrollment timelines, and
contact points, making it easier for families to access services.
By leveraging established relationships with schools, businesses, churches, community organizations, and
existing YMCA members, the program ensures outreach is broad, targeted, and rooted in the community.
This collaborative approach increases awareness, reduces barriers to participation, and helps ensure that
El Mirage residents most in need are informed of and engaged with available childcare and early learning
support services.

Part 3: Program or Project Goals and Outcomes
Please provide up to three goals of the program or project and describe how progress will be
measured to ensure success in providing health and human services to the residents of El
Mirage. It is important to ensure that your goals correspond with the proposed number of residents to
be served within the program year. Explain how you will collect and report data and measure the
impact for each stated goal. 
Example Goal: Of the 80 persons served by the project, 40% (32 persons) will find employment within
three months of entering the program. This will be measured by follow-up contact with each program
participant.

Is program or project implementation contingent upon receiving the full award?
No. The Northwest Valley Family YMCA will continue to operate its childcare programs using existing
resources, including existing scholarships, and other funding sources. However, partial or reduced funding
will limit the number of El Mirage families who can receive tuition assistance. Full funding will allow the
YMCA to serve a greater number of low- and moderate-income households and provide more consistent
support to families with the highest financial need.
If approved for an amount less than 100% of your request, how will you address the shortfall?  
If awarded less than the requested amount, the YMCA will seek to address the shortfall by leveraging
support from individual and corporate donors who have a history of supporting youth and family programs.
Existing donor relationships and community fundraising efforts will be prioritized to supplement tuition
assistance funding. If needed, scholarship awards may be adjusted to serve as many families as possible
while maintaining equitable distribution based on financial need.

The Tuition Assistance Program is supported through a combination of earned revenue, public funding,
and philanthropic contributions. Program fees paid by participating families account for approximately 43%
of total program funding. These fees are set at a level that balances affordability for families with the
operational costs of providing licensed, high quality childcare.
Childcare subsidies from the Arizona Department of Economic Security (DES) represent approximately
55% of funding and are a critical source of support for eligible low income families. These subsidies help
offset tuition costs for families who qualify based on income and employment status.
The remaining 2% of program funding comes from a combination of individual contributions and grant
support, which are used to provide additional financial assistance to families who do not qualify for DES

How will your organization generate or secure additional funding to sustain services for the
continued operation of this program or project? 
The YMCA will sustain the Tuition Assistance Program through a diversified funding strategy that
combines earned revenue, public funding, and ongoing philanthropic support. This approach allows the
YMCA to maintain program stability while expanding access to families with financial need.
A primary source of continued funding is program fees and Arizona Department of Economic Security
(DES) childcare subsidies, which together support the majority of operational costs. These funding streams

To supplement these sources, the YMCA actively secures individual and corporate donations through
annual giving campaigns, targeted fundraising efforts, and community engagement. These contributions
directly support the YMCA’s scholarship fund, which provides tuition assistance to low- and moderate-
income families. The YMCA maintains long-standing relationships with local donors, businesses, and
foundations and will continue to prioritize these partnerships to sustain and expand financial assistance.
The organization will also pursue additional grant opportunities from local, regional, and national funders
aligned with early learning, childcare access, and family stability. This includes both public and private
funding sources that support community-based services for low-income households.
Through this multi-source funding approach the YMCA is well positioned to sustain the program beyond
the grant period and continue serving El Mirage families.
If the program receives less than the requested amount of funding, what adjustments, if any, would
be made to the program and services provided to El Mirage residents?
The Northwest Valley Family YMCA will continue to operate its childcare programs using existing
resources, including existing scholarships, and other funding sources. However, partial or reduced funding
will limit the number of El Mirage families who can receive tuition assistance. Full funding will allow the
YMCA to serve a greater number of low- and moderate-income households and provide more consistent
support to families with the highest financial need.
