Maricopa County through the Office of the Medical Examiner (MCOME)
And
Dr. Bruce Parks, Independent Contractor

The Agreement originally effective August 27, 2020 is further amended as follows:

The MCOME and Dr. Parks may, upon mutual agreement, extend the term of this contract on an annual
basis for a total contract period not to exceed ten years.

The MCOME will advise Dr. Parks no later than 60 calendar days prior to August 27 of each year of the
MCOME intent to extend the contract. All extensions of the contract will be agreed to in writing and
signed by both parties.

All other terms and conditions of this contract are not affected by this Amendment and remain
unchanged.
