5. MEMORANDUM OF UNDERSTANDING PROVIDER: 
 
Maricopa County Human Services Department, providing services in support of Wildfire-MOU#23-0. 
6. PURPOSE: 
 
Extend the term of the MOU.   
 
Confirming Contractor’s initial diaper allotment in accordance with Wildfire’s review of distribution patterns and 
discussions with Contractor concerning future capacity. 
 
Establish an administration fee for the remaining duration of this MOU. 
7. THE ABOVE REFERENCED MOU IS REQUESTED TO BE AMENDED AS FOLLOWS:  
 
Effective on October 1, 2023, or when all parties have signed, whichever is later:  
 
The termination date of the MOU will be September 29, 2024. 
 
Contractor’s total allotment of diapers to distribute will remain at 701,048. 
 
Wildfire agrees to pay Contractor an administration fee of $4,886.36.
