Mutual Agreement to Terminate Statement(s) of Work
Contexture, a Colorado nonprofit corporation, and
     
(“Participant”) are parties to an HIE Participation Agreement with an effective date of        
(“HIE Participation Agreement”).
Contexture and Participant agree to terminate the following Statements of Work that are 
part of the HIE Participation Agreement (“Terminated SOWs”):

This termination:

Is effective as of the date of the last signature on this document.
2.
Does not terminate, or modify the terms of, any other document that is part of 
the HIE Participation Agreement.
3.
Does not affect the rights or duties of either Contexture or Participant relating to
