TO 
EDUCATIONAL AFFILIATION AGREEMENT 
BETWEEN 
PHOENIX CHILDREN’S HOSPITAL, INC.  
AND 
CHANDLER FIRE DEPARTMENT 
This is the FIRST AMENDMENT to the Educational Affiliation Agreement, by and 
between Phoenix Children’s Hospital, Inc. (“PCH”) and Chandler Fire Department 
(“INSTITUTION”), effective July 1, 2019 (the “Agreement”).  In consideration of the mutual 
promises of the parties, the receipt and sufficiency of which is hereby acknowledged, the 
Agreement is hereby amended as follows: 
A.
TERM: The term of the Agreement shall be extended for one (1) year and will
automatically renew for additional one (1) year terms on each anniversary of its
Effective Date after the initial term.
B.
This First Amendment is effective July 1, 2022.
C.
All other terms of the Agreement shall remain in full force and effect.
