BACKGROUND:

The American Heart Association (“AHA”) and Maricopa County Department of Public Health
South Phoenix Healthy Start Program (“MCDPHSPHSP”) share common priorities and interests
in decreasing Nutrition Insecurity in Arizona.

This MOU seeks to maximize the opportunities and resources of and for the AHA and
MCDPHSPHSP to fight hunger. It allows the AHA and MCDPHSPHSP to work together towards
improved nutrition security rates with the target population. The MOU is not intended to promote
or endorse any commercial product or services. While we hope to improve the overall health of
the Arizona community through this MOU, we will focus our activities and quality improvement

The AHA will provide the following support to MCDPHSPHSP:

•
Support your efforts in providing $4,000 for gift cards to local grocery stores to help with
nutrition insecurity.

MCDPHSPHSP will participate in the Nutrition Insecurity program in the following ways:

•
Utilize the support and resources provided to support nutrition insecurity efforts.

EVALUATION OF RELATIONSHIP

The AHA and MCDPHSPHSP will jointly evaluate the effectiveness of this relationship at the
end of each year based on mutually agreed upon criteria in order to establish its future direction,
including but not limited to, effective collaboration on nutrition insecurity improvement. If the
parties determine a desire to expand or modify the scope or duration of activities in support of
the movement, a discussion regarding the appropriateness of an agreement for sponsorship or
corporate promotions will ensue.
