Revised Sections in Scope of Work
3.2.7 CONSULTANT shall maintain close contact with Contractor and keep
continuously aware of the current schedule. CONSULTANT shall keep
Contractor aware of the inspection and testing requirements and the effect
these have on work scheduling, in order to avoid delays and
misunderstandings.
Revise Paragraph 3.2.7 to read:
CONSULTANT shall prepare a Project Materials Sampling and Testing Plan in
accordance with Appendix A19 of the Construction Management Manual and
submit to COUNTY for review. Perform all quality assurance and acceptance
testing and reporting for the PROJECT.
3.2.8 The MCDOT Materials Branch will perform the majority of quality assurance
and acceptance testing, excepting testing of the reinforcing steel
(555.02100A), and the Cement Stabilized Alluvium (222.01000), for which the
CONSULTANT will perform Quality Assurance testing. CONSULTANT shall
coordinate with the MCDOT Lab personnel, and give sufficient advanced
notice (48 hours minimum) of required testing. Utilization of the MCDOT
Materials Laboratory Work Request Form is required. All samples shall be
obtained in accordance with the MCDOT materials procedures or applicable
ASTM or AASHTO sampling methods. Ensure Certificates of Compliance
and/or Certificates of Analysis of material is provided and in compliance with
requirements.

incorporated into the Project in accordance with the ADOT Materials Testing
Manual – Series 900, Appendix C, Table 5, and the Project Materials Sampling
and Testing Plan. Testing is in addition to the Certificates of Compliance.

Ms. Ruane:
Please find attached for your consideration our revised fee proposal for Change Order No. 1 to provide
construction materials field and laboratory testing on the above referenced project, as requested by
MCDOT.
In preparing our fee proposal, we have addressed the comments in your emails dated March 29, 2023 and
August 8, 2023, as well as our phone conversation on August 4, 2023. The attached fee proposal includes
the following:
•
Materials Coordinator – Quality Testing (QT): additional hours for this position to reflect the
increased workload coordinating all field sampling, testing, sample delivery, etc.
•
Laboratory Testing – Quality Testing (QT): all required lab testing billed at QT’s standard rates.
•
Construction Materials Tester – Tristar: one full-time position during peak construction activities,
currently anticipated to be 17 months.
•
Sample Runner – Tristar: one part-time position, approximately 5 hours per week, during peak
construction activities to aid in delivering samples to the QT Laboratory for testing.
•
Girder Inspector/Tester – Consultant Engineering, Inc. (CEI): one additional full-time position
during girder fabrication, which is anticipated to be a 24-hour per day operation.
An overtime allowance was not provided separately by MCDOT in our original contract, as indicated by
the standard note on Table D. Therefore, an allowance equivalent to approximately 10% of all eligible
hours for hourly field personnel has been included with this request in Table A for both Tristar and CEI.
We understand that prior approval of the MCDOT Project Monitor is required prior to using these
allowances. An updated Table D is included for Tristar, CEI, and QT.

Engineering and Management, Inc. for the Maricopa County Department of Transportation Contract No. 2022-001,
Gilbert Road Bridge at Salt River, Work Order TT0339.
Attached, please find our additional fee proposal as requested depicting the additional inspection needed for girder
fabrication testing/inspection. Please note these additional hours are based on previous work experience of similar
scope; full time for approximately 4 months. Also included, we have shown an overtime allowance for our Lead
Inspectors and Girder Inspector/Tester. We’ve also included an updated Table D that was shown in correctly in our
original cost proposal, dated December 1, 2022. Our Proposed Rate for Classification has included a COLA average
over the anticipated three-year contract.

Change Order Request #1 – ¾ Time Materials Coordinator Plus Laboraory
Quality Testing, LLC (QT) – Cost Proposal
Dear Mr. Hall:
Based on the revised scope of work provided by MCDOT, which includes our team now providing all field
testing for the project and performing lab testing for all soils, concrete and asphalt samples at QT’s
laboratory, a revised laboratory testing estimate has been developed. Previously this change order request
was submitted with the material coordinator position increased to full-time. As previously discussed, hours
for the materials coordinator and other positions have been further refined by using the to-date hours
expended for each position plus additional hours budgeted to the end of construction. The following
revisions have been incorporated into this request:
All hours included on this cost proposal are based on actual hours expended through August 4,
2023, plus additional hours budgeted to the end of construction.
Hours for the material manager position have been reduced from 320 hours to 148 hours. This
reduction in hours is justified by the increased materials coordinator hours.
The materials coordinator position is estimated to be ¾-time for the remainder of the
construction schedule.
Direct labor hours for the traffic control specialist position have been reduced from 16 to 6 hours
per month for the remainder of the project. This reduction in hours is based on the actual
workload for this position to-date.
Additional field technician time has been included to cover additional field testing not originally
included in our scope.
Additional laboratory testing has been included for work not originally included in our scope.
Behind this cover letter, please find our revised fee proposal (Tables A, B and C) and our detailed estimate
for laboratory testing. Please let me know if there are any questions regarding this change order request.
