RESOLUTION NO. HB-016 A RESOLUTION OF THE CHANDLER HEALTH CARE BENEFITS TRUST BOARD APPROVING PREMIUM LEVELS FOR PARTICIPANTS IN THE CHANDLER HEALTH CARE BENEFITS PLAN FOR CALENDAR YEAR 2026 BE IT RESOLVED by the City of Chandler Health Care Benefits Trust Board as follows: In accordance with the Chandler Health Care Benefits Trust Fund, created by the City of Chandler (“the City”) through Ordinance No. 4245, adopted on August 16, 2010 (“the Ordinance”), and pursuant to the Chandler Health Care Benefits Trust Agreement, approved through Resolution No. 4447, adopted on August 16, 2010, and executed as of August 17, 2010 (the “Trust Agreement”), as amended by Resolution No. 4571, adopted on January, 10, 2012, and executed as of January 12, 2012, and with the concurrence of the City Manager, the City of Chandler Health Care Benefits Trust Board (the “Trust Board”) hereby approves premium levels for participants in the Chandler Health Care Benefits Plan for Calendar Year 2026. Those capitalized terms not otherwise defined herein shall have the same definitions as set forth in the Ordinance and/or the Trust Agreement. Section 1. Employee Premiums. Monthly Rates for Employee Premiums have been determined by the Trust Board pursuant to Article V of the Trust Agreement. The total monthly premiums for the City’s three alternative health care benefits plans, the “Red Plan,” the “Blue Plan,” and the “White Plan are as follows: Total Monthly Premiums Total Monthly Premiums Total Monthly Premiums Red Plan Blue Plan White Plan Employee Only $1,012.84 $851.20 $680.98 Employee and Spouse $1,691.34 $1,421.45 $1,137.17 Employee and Children $1,498.85 $1,259.65 $1,007.76 Family $2,460.98 $2,068.30 $1,654.66 PASSED AND ADOPTED by the City of Chandler Health Care Benefits Trust Board on this 13th day of August 2025.
