A minimum annual amount of ($ 483,330.15 ) is allocated to this contract based on a percentage of the average spending of the last three (3) years. Additional funds may be available during the year based on a six (6) month budget trend review by the Home Visiting Program Manager and HRPP Manager. Setting a minimum amount with a review at six (6) months allows the Arizona Department of Health Services the opportunity to maximize funding by allocating funds based on the services being provided and will reduce administrative burden by reducing the number of change orders your agency receives throughout the year. Thank you for your continued collaboration and dedication to the High Risk Perinatal Program. The contract period is from July 1, 2023 to June 30, 2024.
