The above-named contract is hereby amended as specified below: A: Attachment A, Scope of Work is being replaced with the attached updated Scope of work beginning on page 2. B: Attachment B, Pricing Sheet, TOTAL AMOUNT is changed to $208,743.70. The additional $82,859.70 is for program expansion and related expenses as described on page 4. II. All other terms and conditions of the original contract shall remain in full force and effect. Staffing: The CHW will provide capacity to communicate with community members in culturally appropriate languages. a. The CHW(s) will be identified as project assignee by APCA and will be required to attend required training as described below. Completed b. Requires Knowledge, Skills, and Attitudes: i. CHW must be able to provide accurate health education using motivational interviewing techniques to patients in a culturally competent manner, using appropriate and accurate language translation with cultural sensitivity and humility, while respecting patient confidentiality and patient autonomy. Ongoing, needs expansion ii. CHW must be willing and able to work effectively with provider partners identified under this grant program. Ongoing, needs expansion 2. Training: A virtual content library for this project is in development and will be required training for CHWs Ongoing, needs expansion a. Training expenses will not exceed $1,000 total. b. Training subjects will include COVID-19, technology tools, and workflows. c. Other trainings may be deemed necessary for the success of this project by the project leadership. d. Initial training will be provided by MCDPH project team. e. A “Train the Trainer” model will be utilized. f. APCA is responsible to ensure any CHW that is unable to attend the initial training is trained by their internal trainer. g. All subjects will be made available via a virtual content library. Amendment: ● Add $13,283 – for additional training to increase CHW capacity and expand network connections. Includes: o $2,164 for travel for two staff/CHW; o $4,500 Training (core competency); o $5,000 CHW Advanced Training (train the trainer); o $1,619 local conference attendances for 10 staff/CHWs ● Add $50,000 – CHW Advance training on topics related to health disparities and support for refugee, immigrant, and migrant communities. This cost includes training supplies and materials and the instructor for training delivery. 3. Incentives: a. Participation incentives are authorized for this project. b. May be eligible for incentives up to $3,000 based on achieving outcome measures assessed by Maricopa County Department of Health and the availability of funds. In progress c. Will be administered at the discretion of APCA’s Chief Executive Officer in support of assigned activities. Project Objectives. 4. Objective #1- Provide CHW workforce support for: Ongoing, needs expansion a. Community events where COVID-19 vaccine hesitancy is identified as a barrier to COVID-19 vaccinations. b. Reinforcing health education relative to COVID-19 testing and mitigation and related public health needs. Navigation to resources needed to adhere to COVID-19 public health efforts. d. Provide appropriate workforce (up to 1 FTE CHW) to support community events as assigned. e. CHW may perform telephonic and/or SMS based outreach on behalf of the FQHC to promote COVID-19 vaccines, screenings, and other delayed care. f. CHW will attend COVID-19 events to assist with community member navigation. g. CHW will provide reminders for community members for upcoming COVID-19 events. h. CHW should be skilled at community engagement and bi-lingual in both English and a second language. Amendment: ● Add $4,640 – for the purchase and service of 3 cell phones for Community Health Workers supporting non-English speaking refugee, immigrant, and migrant populations who need translation or have English language limitations. ● Add $1,404 – for office space for CHWs ● Add $2,500 – for project supplies 5. Objective #2 – Collect and report data from program participants pertinent to project objectives; action referrals to improve community health. Ongoing, needs expansion a. APCA is required to enter all agreements needed with partner FQHC providers under this grant to share PHI, receive referrals, and report back to provider. b. APCA will establish a HIPAA compliant secure bidirectional link to partner FQHC providers to share patient referrals and follow up information. c. APCA is required to provide upon request, de-identified data regarding individuals served under this grant. d. Data collection methods will abide by federal, state, & local governmental guidelines including confidentiality and ongoing Covid-19 requirements. e. Explicit instructions will be published to grantee that cover the method, IRB clearance and coordinating instruction pertaining to technical requirements that govern community- based research. Amendment: ● Add $3,500 – for PDSA Implementation, anticipated costs based on PDSA meeting implementation. This may include development and deployment of communication and marketing materials (graphics, memes, etc), translation, interpretation services, or other supportive resources that emerge from the project leadership and partners. IV. DELIVERABLES/SERVICES: Deliverables associated with this grant are described below. a. Outreach: CHW will perform outreach to patients identified by provider partners via a secure communication device. b. Navigation: CHW will navigate individuals as required to community based c. Follow up: Follow up will be provided to community members as the need is identified. A written report of follow-up activities will be required to document the services provided to the community. d. Event support: Assignment to support for events will be identified by the grantor, according to this agreement. Events will focus on community events that focus on the vaccination of those community members who are of low socio-economic status and will be assigned in advance. e. Data points: Data from community members who are serviced will be collected within a prescribed workflow. f. Data collection: Data collection will be done electronically. Information will be housed in accordance prescribed workflow.
