37. AMENDMENT TO SUBLEASE AGREEMENT WITH A NEW LEAF, INC. Approve and execute non-financial Amendment No. 5 to the Sublease Agreement (L-7451) between Maricopa County (Sublessee) and A New Leaf, Inc. (Sublessor). The County utilizes 2,779 square feet located at the La Mesita Family Homeless Center, 2254 West Main Street, Mesa, Arizona, to operate 2 Early Head Start and 1 Head Start classrooms. The purpose of this Amendment No. 5 is to address the following: A. Revises Article 6 (Term and Termination), by removing and replacing subsections and extending the term of the Agreement for three (3) years commencing on July 1, 2024, through June 30, 2027. Adding subsection 6.5, which lists A.R.S. §38-511. B. Revises Article 17 (Authority to Execute Agreement/Counterparts), by removing subsection 17.1 in its entirety and replacing with revised language that changes the word Sublease to Agreement. Subsections 17.2 and 17.3 are being added, that detail administer duties and counterparts with electronic signatures. There is no cost to the County for this lease renewal. 39. AMENDMENT TO SUBLEASE AGREEMENT (L-7451) WITH A NEW LEAF Approve and execute non-financial Amendment No. 6 to Sublease Agreement between Maricopa County (County/Sublessee) and A New Leaf, Inc., dated May 21, 2014, as amended (Agreement), for County’s use of premises located at 2254 West Main Street, Mesa, Arizona for Head Start services. There is no cost to County for this lease. The purpose of the Amendment is to add the option of Sublessee to conduct tenant improvements as needed, at Sublessee’s expense and, adds the playground and splashpad to the subleased property. The Amendment extends the term of the Agreement through June 30, 2032. Supervisory District: 2 (C-22-14-067-1-06)
