30. AMENDMENT TO MOU WITH CARE1ST HEALTH PLAN OF ARIZONA REGARDING COLLABORATIVE PROTOCOL Approve Amendment Number 1 between Care1st Health Plan of Arizona and Maricopa County through the Department of Correctional Health Services (MCDCHS). This approval extends to subsequent Amendments to this MOU which extend the time frame or amend the name of the organization. Such amendments shall be sent to the Clerk of the Board with a recommendation of approval by the MCDCHS Appointing Authority or his/her designee and a copy of this agenda. The Clerk’s Office will sign all such documents, retain a copy for the official records and return a signed copy to the department. The purpose of this Amendment is to extend the term for a twelve (12) month period. This Amendment is non-financial and is effective from May 8th, 2020 until May 7th, 2021. Conforming language regarding subsequent amendments will be added to the signature documents. All other terms and conditions of this Agreement remain in full force and effect. (C-26-19-020-3-01)

37. AMENDMENT TO MOU WITH CARE1ST REGARDING COLLABORATIVE PROTOCOL Approve Amendment No. 3 to the Memorandum of Understanding (MOU) between Care1st Health Plan of Arizona, a Wellcare Co. and Maricopa County, by and through Maricopa County Correctional Health Services, for the purpose of establishing a collaborative protocol for effective communication, coordination, and continuity of care for individuals eligible for services provided by Care1st who are also served by Maricopa County Correctional Health Services (MCCHS). This amendment retroactively renews the MOU for a term from May 8, 2022 ending May 7, 2023. This Memorandum of Understanding is non-financial. (C-26-19-020-3-03)
