43. AGREEMENT WITH COMMUNITY MEDICAL SERVICES HOLDINGS, LLC Approve an agreement between Community Medical Services Holdings, LLC "CMS" and Maricopa County to establish a program to increase access to Medication Assisted Treatment (“MAT”), Opioid Use Disorder (“OUD”) recovery support services, and opioid prevention awareness for incarcerated patients transitioning to the community. This Agreement shall begin upon approval by the Maricopa County Board of Supervisors and shall terminate one year thereafter unless otherwise terminated or amended as provided herein. By mutual written amendment executed by the Parties’ authorized representatives, this Agreement may be extended for supplemental periods of twelve (12) months, up to maximum of forty-eight (48) months. There are no financial responsibilities assigned by this Agreement; the Agreement is intended to allow CMS and Correctional Health Services (CHS) to work together to better address the needs of the community. Nothing in this Agreement shall be deemed to be a commitment or obligation of funds, personnel, or other resources by either party. (C-26-23-014-X-00)

36. AMENDMENT TO AGREEMENT WITH COMMUNITY MEDICAL SERVICES Approve Amendment No. 2 to the Agreement between Community Medical Services Holdings, LLC ("CMS") and Maricopa County by and through its Department of Correctional Health Services (CHS). This agreement aims to establish a program aimed at increasing access to Medication Assisted Treatment (MAT), Opioid Use Disorder (OUD) recovery support services, and raising awareness about opioid prevention for individuals transitioning from incarceration to the community. The purpose of this amendment is to renew for a two-year term of twenty-four (24) months. This non-financial amendment is effective from March 1st, 2025, and ending February 28th, 2027.
