54. AMENDMENT TO LEASE AGREEMENT WITH A NEW LEAF, INC. Approve and execute Amendment No. 2 to Lease Agreement No. L-7444 with A New Leaf, Inc., an Arizona non-profit corporation (“Landlord”), for approximately 5,000 square feet of office and clinic space located at 635 E Broadway Rd, Mesa, AZ 85204. Parties have agreed to amend the Agreement to provide for lease administration authority and for security service shared costs. In addition to basic rent and the prorated Monument Sign cost, Lessee shall pay Lessor a monthly fee of Two Thousand Five Hundred Dollars ($2,500.00) for Lessee’s shared cost for on-site security services, effective October 1, 2021. The current five-year term expires on December 31, 2023. County has the right to terminate the Agreement upon ninety (90) days’ prior written notice. All other terms and conditions of the Agreement remain the same and in full force and effect. The WIC program provides assistance to qualified low-income women and children of nutritional risk including providing supplemental foods, education, community referrals and other preventive services including, but not limited to oral health screening for low-income pregnant, breast feeding and non-breast-feeding post-partum women and for infants and children up to five years of age found to be at nutritional risk. This action takes place in Supervisorial District No. 2 (C-86-14-055-1-03)
