70. TERMINATION OF FACILITIES USE/REVOCABLE LICENSE AGREEMENT Approve termination of that certain Facilities Use/Revocable License Agreement between Maricopa County (County) and Phoenix Day (Contractor) dated October 15, 2015, as amended (Agreement) and delegation of signature authority to the Real Estate Director to execute the notice letter to terminate the Agreement. Pursuant to the Agreement, County’s lett er provides Contractor with thirty (30) days’ prior written notice to terminate the Agreement on June 30, 2023. County no longer operates County Public Health WIC facilities located at 8828 N. Central Ave., Phoenix, AZ, 85020 and 5422 W. Thunderbird Rd, Gl endale, AZ, 85306 (“Premises”). The licensed properties are in Supervisor Districts 3 and 4 (C-86-16-018-M-03)
