AMENDMENT TO IGA WITH DYSART ELEMENTARY SCHOOL DISTRICT FOR DENTAL SEALANT PROGRAM SERVICES Approve a retro-active non-financial amendment 1 to the Intergovernmental Agreement (IGA) between Dysart Elementary School District and Maricopa County by and through its Department of Public Health, Office of Oral Health Dental Sealant Program for free dental sealant services for the students. This amendment extends the term for one year (1) to begin July 1, 2020 and ending June 31, 2021. All terms and conditions of the original IGA shall remain in full force until terminated by either party. The contract may be extended for additional one (1) year periods, not to exceed a total extended term of five (5) years. This amendment is for year two of the five-year term. (C-86-20-017-3-01)

AMENDMENT TO IGA WITH DYSART UNIFIED SCHOOL DISTRICT FOR DENTAL SEALANT PROGRAM SERVICES Approve Amendment 3 to the non-financial Intergovernmental Agreement (IGA) between Dysart Unified School District and Maricopa County by and through its Department of Public (MCDPH) Health, Office of Oral Health Dental Sealant Program for dental sealant services for the students. This amendment extends the term for one year to begin July 1, 2022 and end June 30, 2023. All other terms and conditions of the original IGA shall remain in full force until terminated by either party. This amendment is for year 4 of the 5-year term. There is the option to extend this agreement one additional year. The purpose of th is IGA is to continue cooperation between MCDPH and Dysart Unified School District to provide dental sealants to eligible students for the prevention of tooth decay. School-based dental sealant programs are evidence-based and provide a safe, inexpensive mo del for delivering sealants to school-age children. Dental sealants are best-practice for reducing tooth decay in the chewing surfaces of permanent molars. (C-86-20-017-3-03)
