93. AMENDMENTS TO CONTRACTS WITH UNLIMITED POTENTIAL AND ASIAN PACIFIC COMMUNITY IN ACTION FOR THE PURCHASE OF SERVICES RELATED TO ADVANCING HEALTH LITERACY TO ADVANCE EQUITABLE RESPONSES TO COVID-19 Approve Amendment(s) 1 by and through the Department of Public Health and Sub-Recipient contracts with Unlimited Potential and Asian Pacific Community in Action for the purchase of services related to advancing health literacy to advance equitable responses to COVID-19. I. The above-named contracts are hereby amended as specified below: A. Contract Expiration dates shall be updated to December 31, 2023 B. Term and Conditions shall be updated to include: i. WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-393.01: If Contractor engages in for-profit activity and has 10 or more employees, and if this agreement has a value of $100,000 or more, Contractor certifies it is not currently engaged in, and agrees for the duration of this agreement to not engage in, a boycott of goods or services from Israel. This certification does not apply to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 U.S.C. § 4842. ii. WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-394: Contractor warrants and certifies that it does not currently, and agrees for the duration of the Agreement, that it will not use: 1. the forced labor of ethnic Uyghurs in the People's Republic of China, 2. any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China, 3. any contractors, subcontractors or suppliers that use the forced labor or any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China. If Contractor becomes aware during the term of this Agreement that the Contractor is not in compliance with this paragraph, Contractor shall notify the County within five business days after becoming aware of the noncompliance. If the Contractor fails to provide a written certification to the County that the Contractor has remedied the noncompliance within 180 days after notifying the County of its noncompliance, then the agreement terminates, except that if the agreement termination date occurs before the end of the 180-day period, the agreement terminates on the agreement termination date. II. All other terms and conditions of the original contract shall remain in full force and effect. (C-86-22-061-X-01)

49. AMENDMENT TO CONTRACT WITH ASIAN PACIFIC COMMUNITY IN ACTION FOR THE PURCHASE OF SERVICES RELATED TO ADVANCING HEALTH LITERACY TO ADVANCE EQUITABLE COMMUNITY RESPONSES TO COVID-19 Approve Amendment 2 to Public Health Contract #PH NOI 21021, between Maricopa County (County), a political subdivision of the State of Arizona, through its Department of Public Health (MCDPH) and Asian Pacific Community in Action, (“Contractor”) for services related to advancing health literacy to advance equitable community responses to COVID-19. This amendment increases the total contract amount by $82,859.70 to a new not to exceed $208,743.70. This amendment adds additional services to the scope of work as outlined beginning on page 2 of the amendment. This agreement is being funded by the Advancing Health Literacy to Enhance Equitable Community Responses to COVID-19, a COVID-19 related award. The purpose of this grant is to help implement effective, evidence-based health literacy strategies that are culturally appropriate to enhance COVID-19 testing, contact tracing, and/or other mitigation measures in racial and ethnic minority populations and other socially vulnerable populations, including racial and ethnic minority rural communities. Should the funding source cease and alternate funding not be available, Section 4.0 of the Contract, Availability of Funds, will apply allowing for a change or termination of the contract. This contract was entered into pursuant to MC1-346 of Maricopa County Procurement Code. (C-86-22-061-X-02)
