82. AMENDMENT TO NON-PROCUREMENT CONTRACT #220160 WITH THE ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS FOR FEDERALLY QUALIFIED HEALTHCARE CENTERS Approve an Amendment to the Non-Procurement Contract #220160, between Maricopa County (County), through its Department of Public Health (MCDPH) and Arizona Alliance for Community Health Centers (AACHC) for grant assistance program. The program will continue to support Maricopa County-based Federally qualified healthcare centers and address the need to provide expanded services as a result of the COVID-19 Pandemic. The total cost of the services will increase funding by $27,000,000 where $26,550,000 will fund grant assistance programs and $450,000 for Arizona Alliance for Community Health Center (AACHC) operational expenses through 2026 . ) . The total amount not-to-exceed is $52,000,000. The term of this Contract is four (4) years and eight (8) months. The end date has been extended from December 31, 2024, to June 30, 2026. The contract will be funded through the American Rescue Plan Act (ARPA) and If funds are not available, County may suspend or cancel this agreement. 86. AMENDMENT TO NON-PROCUREMENT CONTRACT #220160 WITH THE ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS FOR FEDERALLY QUALIFIED HEALTHCARE CENTERS Approve Amendment No. 3 for Non-Procurement Contract C-86-22-063-X-00, between Maricopa County, by and through its Department of Public Health (MCDPH) and Arizona Alliance for Community Health Centers (AACHC) for continued support of Maricopa County-based Federally Qualified Healthcare Centers (FQHC), who are providing expanded services as a result of the COVID-19 Pandemic. I. The above-named Contract is hereby amended as specified below: A. Contract Expiration date shall be updated from June 30, 2026 to September 30, 2026. B. Section 3.6 shall be changed to read: “Funds must be obligated (through a purchase order or formal contract) by December 31, 2024. At that time, contractors will have 90 days (until March 31, 2025) to return any unobligated funds to MCDPH. All funds must then be disbursed/expended by September 30, 2026.” C. Terms and Conditions shall be updated to include: i. WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-393.01: If Contractor engages in for-profit activity and has 10 or more employees, and if this agreement has a value of $100,000 or more, Contractor certifies it is not currently engaged in, and agrees for the duration of this agreement to not engage in, a boycott of goods or services from Israel. This certification does not apply to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 U.S.C. § 4842. ii. WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-394: Contractor warrants and certifies that it does not currently, and agrees for the duration of the Agreement, that it will not use: 1. the forced labor of ethnic Uyghurs in the People's Republic of China, 2. any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China, 3. any contractors, subcontractors or suppliers that use the forced labor or any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China. If Contractor becomes aware during the term of this Agreement that the Contractor is not in compliance with this paragraph, Contractor shall notify the County within five business days after becoming aware of the noncompliance. If the Contractor fails to provide a written certification to the County that the Contractor has remedied the noncompliance within 180 days after notifying the County of its noncompliance, then the agreement terminates, except that if the agreement termination date occurs before the end of the 180-day period, the agreement terminates on the agreement termination date. II. All other terms and conditions of the original contract shall remain in full force and effect. (C-86-22-063-X-03)

40. AMENDMENT TO NON-PROCUREMENT CONTRACT #220160 FOR THE ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS Approve an Amendment to the Non-Procurement Contract #220160, between Maricopa County (County), through its Department of Public Health (MCDPH) and Arizona Alliance for Community Health Centers (AACHC) for grant assistance program. The program will continu e to support Maricopa County-based Federally qualified healthcare centers and address the need to provide expanded services as a result of the COVID-19 Pandemic. The above-named contract is hereby amended as specified below: A. Delete Section 3.7 on Page 2 of the original contract and replace with the following revised Section 3.7: 3.7 Expenditure and Performance Reporting shall be submitted as follows: 3.7.1 Quarterly Expenditure and Performance Reports indicating use of funds by category, performance met ric data, and additional reports as identified in Appendix A. 3.7.2 Quarterly Expenditure and Performance Reports will cover periods from (Q1) January 1 to March 31; (Q2) April 1through June 30; (Q3) July 1 through September 30; and (Q4) October 1 through December 31. 3.7.3 Quarterly reports will be due no later than the 21st day of the month prior to the Quarter of submission. 3.7.4 Annual Expenditure and Performance Reports will summarize use of funds and performance metric data for that year. 3.7.5 Annual Expenditure and Performance Reports will be due annually on April 21st along with a preliminary report reflecting all required data, including but not limited to: applicants and recipients, amounts awarded, primary purpose of award, and perfo rmance metric data. B. Clarify Section 10.4 of the original contract to stipulate a total of $1,237,520 is to be maintained by Contractor for administration costs. II. All other terms and conditions of the original contract shall remain in full force and e ffect. (C-86-22-063-X-02)
