85. CONTRACT WITH PROVIDERTECH LLC FOR THE PURCHASE OF SERVICES RELATED TO ADVANCING HEALTH LITERACY TO ADVANCE EQUITABLE COMMUNITY RESPONSES TO COVID-19 Pursuant to MC1-346 of Maricopa County Procurement Code, approve a Public Health Contract #PH NOI 21021, between Maricopa County (County), a political subdivision of the State of Arizona, through its Department of Public Health (MCDPH) and ProviderTech LLC, (“Contractor”) for services related to advancing health literacy to advance equitable community responses to COVID-19. The term of the contract is two (2) years, beginning on July 1, 2021 and ending June 30, 2023. The cost of the services is in a not to exceed amount of $185,170 for the first year and $155,160 for the second year for a combined term total of $340,330. The County may, at its option and with the agreement of the Contractor, renew the term of this Contract for additional terms up to a maximum of three (3) additional years, (or at the County’s sole discretion, extend the contract on a month-to-month basis for a maximum of six (6) months after expiration). AMENDMENT TO CONTRACT #PH NOI 21021 WITH PROVIDERTECH, LLC FOR THE PURCHASE OF SERVICES RELATED TO ADVANCING HEALTH LITERACY TO ADVANCE EQUITABLE COMMUNITY RESPONSES TO COVID-19 Approve Amendment 1 by and through Department of Public Health and ProviderTech, LLC for Contract #PH NOI 21021 for the purchase of services related to advancing health literacy to advance equitable community responses to COVID-19. The above-named contract is hereby amended as specified below: A. Exhibit A, Pricing TOTAL BUDGET ALLOCATION is changed to $352,030. The additional $11,700 is for the additional services beginning on page 2 of this Amendment. All other terms and conditions of the original contract shall remain in full force and effect. AMENDMENT TO CONTRACT WITH PROVIDERTECH FOR THE PURCHASE OF SERVICES RELATED TO ADVANCING HEALTH LITERACY TO ADVANCE EQUITABLE RESPONSES TO COVID-19 Approve Amendment 3 by and through the Department of Public Health and Sub-Recipient contract with Providertech for the purchase of services related to advancing health literacy to advance equitable responses to COVID-19. I. The above-named contract is hereby amended as specified below: A. Contract Expiration date shall be updated to December 31, 2023 II.
