103. REQUEST FOR PROPOSAL # 220177 FOR COMMUNITY HEALTH WORKERS FOR COVID RESPONSE AND RESILIENT COMMUNITIES Pursuant to MC1-329 of the Mar icopa County Procurement Code, authorize the Chairman of the Board of Supervisors to execute a maximum of nine total Contracts to be developed under #PH RFP 220177, Community Workers for Covid Response and Resilient Communities (CCR). The purpose of the funds is to address the socio-economic impact of health disparities in COVID-19 health outcomes, vaccine hesitancy and trust in healthcare for the residents of Maricopa County. Maricopa County Department of Public health will select up to a total of nine qualified Contractors to enter one-year, fixed-price, service contracts. Respondents are free to apply for more than one of the opportunities detailed in the RFP. Resulting contr actors may be a Federally Qualified Health Center, Community Health Worker Organization, or a Refugee & Immigrant Serving Organization. The resulting Contracts are granted a term of twelve (12) months, beginning July 1, 2022 through June 30, 2023, with a n option to renew for four (4) additional years (or at the County’s sole discretion, extend the contract on a month-to-month bases for a maximum of six (6) months after expiration) based on an updated Scope of Work. The approximate total amount to be spent on this service is $1,648,345 for the initial one-year period. This agreement will be funded by the Health Literacy Grant (HL Grant # 1 CPIMP211277-01-00). 94. AMENDMENTS TO CONTRACTS WITH WESLEY COMMUNITY AND HEALTH CENTERS, INTERNATIONAL RESCUE COMMITTEE AND HELPING FAMILIES IN NEED FOR COMMUNITY HEALTH WORKERS FOR COVID RESPONSE AND RESILIENT COMMUNITIES (CCR) Approve Amendment(s) 1 by and through the Department of Public Health and Sub-Recipient contracts with Wesley Community and Health Centers, International Rescue Committee and Helping Families In Need for community health workers for COVID response and resilient communities (CCR). I. The above-named contracts are hereby amended as specified below: A. Contract Expiration dates shall be updated to December 31, 2023. B. Term and Conditions shall be updated to include: i. WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-393.01: If Contractor engages in for-profit activity and has 10 or more employees, and if this agreement has a value of $100,000 or more, Contractor certifies it is not currently engaged in, and agrees for the duration of this agreement to not engage in, a boycott of goods or services from Israel. This certification does not apply to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 U.S.C. § 4842. ii. WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-394: Contractor warrants and certifies that it does not currently, and agrees for the duration of the Agreement, that it will not use: 1. the forced labor of ethnic Uyghurs in the People's Republic of China, 2. any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China, 3. any contractors, subcontractors or suppliers that use the forced labor or any goods or services produced by the forced labor of ethnic Uyghurs in the people's Republic of China. If Contractor becomes aware during the term of this Agreement that the Contractor is not in compliance with this paragraph, Contractor shall notify the County within five business days after becoming aware of the noncompliance. If the Contractor fails to provide a written certification to the County that the Contractor has remedied the noncompliance within 180 days after notifying the County of its noncompliance, then the agreement terminates, except that if the agreement termination date occurs before the end of the 180-day period, the agreement terminates on the agreement termination date. II. All other terms and conditions of the original contract shall remain in full force and effect. (C-86-22-172-X-01)
