71. PH RFP 230126 SUPPORTING MARICOPA COUNTY’S AGING AND OLDER ADULTS’ POPULATION CONTRACTS Pursuant to MC1-329 of the Maricopa County Procurement Code, authorize the Chairman of the Board of Supervisors to execute one or more Public Health Request for Proposal (PH RFP) contracts, in a form approved by the County Attorney’s Office, to one or more successful bidders in response to PH RFP230126 Supporting Maricopa County’s Aging and Older Adults' Population. The goal is to contract with agencies serving older adults to identify new opportunities or expand existing initiatives. Initiatives may include building systems/networks/partnerships that address social isolation because of COVID; enhancing technology and virtual connection to increase access to telehealth; addressing vaccine hesitancy or increasing COVID-19 vaccination rates; strengthening agency infrastructure by building an inclusive workforce; increasing access to care and mental health using telehealth, home visitation, ambulatory care, community health workers, etc.; educating and preparing a better system to serve the aging population in case of a health emergency such as a pandemic; updating data infrastructure to better track key demographics; and, updating communications/processes/systems to be culturally and linguistically appropriate. The RFP was released on February 01, 2023, and responses are due by 2:00pm on March 10, 2023. A Public Health Evaluation Committee will review all responses received utilizing evaluation criteria included in the RFP and select those most advantageous to Maricopa County. The anticipated amount to be spent among all awarded contracts is approximately $1M with a limit of $500,000 per entity. The anticipated start date of the contracts is April 1, 2023 and will end May 31, 2024. If additional funds become available, awards resulting from this solicitation may be amended and/or extended based on the need for continuous services as well as satisfactory Contractor performance. These agreements will be funded by the COVID-19 Health Improvement Plan-Disparities Grant. Should the funding source cease and alternate funding not be available, Section 4.2 of the RFP, Availability of Funds, will apply allowing for a change or termination of the agreement. AMENDMENT TO CONTRACTS WITH THE CLIPDART GIVEBACK, ARIZONA CAREGIVER COALITION, BENEVILLA, KOREAN AMERICAN NURSES FOUNDATION, CANCER SUPPORT COMMUNITY ARIZONA, VALLEY OF THE SUN YMCA, CITY OF TEMPE, AND OAKWOOD CREATIVE CARE Approve Amendment 1 to PH RFP 230126 between Maricopa County by and through the Department of Public Health and The ClipDart Giveback, Arizona Caregiver Coalition, Benevilla, Korean American Nurses Foundation, Cancer Support Community Arizona, Valley of the Sun YMCA, City of Tempe, and Oakwood Creative Care. The amendment for The ClipDart Giveback shall update the term expiration date to December 31, 2024, to fully expend funds and report on deliverables as well as all invoices must be received by January 31, 2025. The amendment for Arizona Caregiver Coalition shall update the term expiration date to August 31, 2024, to fully expend funds and report on deliverables as well as all invoices must be received by September 30, 2024. The amendment for Benevilla shall update the term expiration date to June 30, 2024, to fully expend funds and report on deliverables as well as all invoices must be received by July 31, 2024. The amendment for Korean American Nurses Foundation shall update the term expiration date to December 31, 2024, and includes a new disbursement matrix shown on page two (2) of the amendment. 60. CONTRACT AMENDMENT WITH THE CITY OF TEMPE TO SUPPORT THE AGING AND OLDER ADULTS POPULATION Approve Contract Amendment 2 to PH RFP 230126 between Maricopa County by and through the Department of Public Health and The City of Tempe. This amendment shall reduce the contract from $499,083 to $406,458 and includes a new Exhibit A-1 on page 2 of the amendment. The revised not to exceed (NTE) shall be updated to $406,458. This amendment also rescinds Section B from Item 1 of Amendment 1 which serves to remove the disbursement schedule. All other terms and conditions of the original contract shall remain in full force and effect. (C-86-23-102-X-02)
