77. AMENDMENT TO CONTRACT WITH KEYS TO CHANGE Approve Contract Amendment 3 to PH RFP 240052 between Maricopa County by and through the Department of Public Health and Keys to Change. This amendment shall update the term expiration date to August 31, 2026. Exhibit A, Pricing Compensation/Fees & Total Budget shall be updated to include an additional $385,000, $225,000 of which is funded by Arizona Department of Health Services (ADHS) that must be spent by September 29, 2025, bringing the new not to exceed (NTE) to $745,000. Exhibit B, Scope of Work, shall be replaced with the Year 3 Revised Scope of work. Exhibit G, Additional Terms and Conditions shall be added for the Transitional Living Services, operating expenses that are funded by Arizona Department of Health Services (ADHS). All other terms and conditions of the original contract shall remain in full force and effect. This contract was completed pursuant to MC1-329 of the Maricopa County Procurement Code. Funding for this contract is from the U.S. Department of Health and Human Services, Centers for Disease Control (CDC), “Overdose Data to Action: Limiting Overdose through Collaborative Actions in Localities (OD2A: LOCAL) grant for services related to overdose surveillance, recovery, and care in communities most impacted by the overdose crisis. This contract assists Maricopa County in helping to limit overdoses through collaborative actions supporting efforts to limit fatal and non-fatal overdoses with a special consideration for groups disproportionately affected by overdoses.
