61. AMENDMENT FOR SUBSTANCE USE BUSINESS TOOLKITS Approve Amendment 1 to contract PH RFP 250104 between Maricopa County, by and through the Department of Public Health, and Valley Leadership Corporation. Section 1.0, Contract Term, shall be updated to reflect a new expiration date of June 30, 2027. Exhibit A, Budget Detail and Narrative, shall be updated with the addition of Exhibit A-2 Year 2 Budget Sheet & Narrative. An additional $349,998 shall be added for the budget period of July 1, 2026 – June 30, 2027, making the new not to exceed (NTE) $699,996. Exhibit C-2, Year 2 Scope of Work, shall be added for services from July 1, 2026 – June 30, 2027. This amendment includes the addition of Exhibit E, Year 2 Renewal Form, and Exhibit F, Year 2 Work Plan & Timeline. All other terms and conditions of the original contract shall remain in full force and effect. The purpose of the contract is supported by the One Arizona Agreement, aiming to expand the adoption and impact of an Opioid Business Toolkit that assists businesses in developing and implementing workplace policies addressing opioid use disorder (OUD). Businesses play a vital role in the community, as the effects of opioid misuse extend beyond individual employees to their families and the broader workforce. Workplace policies that reduce stigma, support employees, and connect individuals to care contribute to healthier, more resilient communities. (C-86-26-041-X-00)
