DATA USE AGREEMENT WITH THE CENTERS FOR DISEASE CONTROL AND PREVENTION Approve the non-financial Data Use Agreement (DUA) between Maricopa County, by and through its Department of Public Health (MCDPH), and The Centers For Disease Control and Prevention (CDC). This agreement sets forth the terms and conditions under which MCDPH will permit CDC to receive and use data related to Project Q of the ELC Grant. This data is related to Antimicrobial Resistance (AR) surveillance and Sexually Transmitted Infections (STI’s). The term of this Agreement shall commence upon signature of both parties (“Effective Date”) and shall continue for a period of five (5) years, unless earlier terminated or canceled. This Agreement may be renewed upon mutual written consent of the Parties. (C-86-26-044-X-00)
